COOK双球囊联合缩宫素对妊娠晚期产妇的影响研究

The effect of COOK double balloon combined with oxytocin on pregnant women in the third trimester

目的:探讨COOK双球囊联合缩宫素对妊娠晚期产妇宫颈成熟度、引产效果及血清TNF-α、IL-8和IL-1β水平的影响。方法:纳入本院收治的妊娠晚期产妇60例,纳入时间为2018年3月至2020年3月,采用数字表法分为对照组以及研究组,对照组给予缩宫素干预,研究组在对照组的基础上给予COOK双球囊干预,对比两组患者引产结果、宫颈成熟度、血清TNF-α、IL-8和IL-1β水平。结果:干预前,两组宫颈成熟度无显著差异(P>0.05),干预后,宫颈成熟度均显著增加(P<0.05),干预后研究组宫颈成熟度高于对照组(P<0.05)。研究组的自然分娩率(90.00%)高于对照组(56.67%),剖宫产率(10.00%)低于对照组(43.33%)(χ2=8.522,P<0.05)。研究组产后出血、胎儿窘迫、胎儿窒息等不良妊娠结局发生率(3.33%)低于对照组(26.67%)(χ2=66.405,P<0.05)。研究组的总产程时长、产后出血量均低于对照组,研究组新生儿Apgar评分高于对照组(P<0.05)。干预后,研究组的TNF-α、IL-8和IL-1β水平低于对照组(P<0.05)。结论:COOK双球囊联合缩宫素能有效促进妊娠晚期产妇宫颈成熟,降低炎症反应,改善妊娠结局。

Objective:To investigate the effects of COOK double balloon combined with oxytocin on cervical maturity,induced labor and serum TNF-α,IL-8 and IL-1βlevels in late pregnancy women.Methods:60 cases of late pregnancy women admitted to our hospital from March 2018 to March 2020 were divided into control group and study group by digital table method.The control group was given oxytocin intervention,and the study group was given COOK double balloon intervention on the basis of the control group.The induced labor results,cervical maturity,serum TNF-α,IL-8 and IL-1βlevels of the two groups were compared.Results:There was no significant difference in cervical maturity between the two groups before the intervention(P>0.05),and cervical maturity increased significantly after the intervention(P<0.05),the cervical maturity after the intervention in the study group was higher than that in the control group(P<0.05).The natural delivery rate of the study group(90.00%)was higher than that of the control group(56.67%),and the cesarean section rate(10.00%)was lower than that of the control group(43.33%)(χ2=8.522,P<0.05).The incidence of postpartum hemorrhage,fetal distress,fetal asphyxia and other adverse pregnancy outcomes in the study group(3.33%)was lower than that in the control group(26.67%)(χ2=66.405,P<0.05).The total duration of labor and postpartum hemorrhage in the study group were lower than those in the control group,and the Apgar score of newborns was higher than that in the control group(P<0.05).After intervention,the levels of TNF-α,IL-8 and IL-1βin the study group were lower than those in the control group(P<0.05).Conclusion:COOK double balloon combined with oxytocin can effectively promote cervical maturation,reduce inflammation and improve pregnancy outcome.
