2020—2021年某院抗菌药物合理应用管理情况探究

Research on the Management of Rational Application of Antibiotics in a Hospital from 2020 to 2021

目的调查某院2020—2021年抗菌药物合理使用情况,提出相应的管理对策。方法选取某院2020—2021年1000张电子处方(含2020年度500张、2021年度500张),对1000张电子处方中的抗菌药物进行统计分析,包括统计抗菌药物的使用率、金额、用药方式、给药途径、DDDs排序、不合理使用情况等。结果某院2020—2021年抗菌药物使用百分率、平均抗菌药物金额、占处方总金额的百分比均出现逐年下降的趋势,差异有统计学意义(P<0.05);抗菌药物单用比例为57.00%,口服给药的比例为39.00%;DDDs前5位抗菌药物中,主要包括头孢西丁钠、左氧氟沙星、头孢曲松钠、青霉素钠、头孢他啶,有多数品种DUI≤1;抗菌药物不合理使用原因分别为用药时间过长、未送检病原学检查、用法用量不合理、药物联用不合理,占比分别为3.00%、1.00%、1.90%、2.10%,总占比为8.00%。结论某院2020—2021年还存在一些抗菌药物不合理使用现象,需不断加强人员培训、制度管理及监管力度。

Objective To investigate the rational use of antibiotics in a hospital from 2020 to 2021,and to propose corresponding management countermeasures.Methods 1000 electronic prescriptions(including 500 in 2020 and 500 in 2021)of a hospital from 2020 to 2021 were selected.Statistical analysis of antimicrobial drugs in 1000 electronic prescriptions,including usage rate,amount,medication use,route of administration,ranking of DDDs,and irrational use of antimicrobial drugs.Results The percentage of antibiotics used,the average amount of antibiotics,and the percentage of the total amount of prescriptions in a hospital from 2020 to 2021 all showed a downward trend year by year,and the difference was statistically significant(P<0.05).The proportion of antibiotics alone was 57.00%,and the proportion of oral administration was 39.00%.Among the top 5 antibacterial drugs of DDDs,mainly included cefoxitin sodium,levofloxacin,ceftriaxone sodium,penicillin sodium,ceftazidime,and most varieties had DUI≤1.The reasons for the irrational use of antibiotics were that the medication time was too long,the etiological examination was not submitted,the usage and dosage were unreasonable and the combination of drugs was unreasonable,accounting for 3.00%,1.00%,1.90%,and 2.10%,the total proportion was 8.00%.Conclusion There are still some irrational use of antibacterial drugs in a hospital from 2020 to 2021.It is necessary to continuously strengthen personnel training,system management and supervision.
