摘要
目的探讨类风湿性关节炎诊断中采用动态增强MRI联合64排螺旋CT的价值。方法50例类风湿性关节炎患者均行动态增强MRI与64排螺旋CT检查,分析动态增强MRI单一检查(对照组)、动态增强MRI联合64排螺旋CT检查(观察组)价值,并就各方案对侵蚀程度的影像特征进行分析。结果①检出率:50例患者共检出阳性关节60个,观察组为93.33%、对照组为83.33%,组间对比P<0.05。②准确率:以实验室检查结果为金标准,观察组(95.00%)高于对照组(81.67%),组间对比P<0.05。③侵蚀程度的影像特征判断:MRI侵蚀程度评分为2.14±0.42分,CT侵蚀程度评分为0.56±0.11分,组间对比差异显著P<0.05。结论动态增强MRI联合64排螺旋CT在类风湿性关节炎诊断中应用,不仅可以提高诊断准确率,还可准确判断患者侵蚀程度,为临床诊治提供了参考,值得借鉴。
Objective To explore the clinic value of dynamic enhanced MRI combined with 64 slice spiral CT in the diagnosis of rheumatoid arthritis.Methods 50 patients with rheumatoid arthritis were included in the study,and dynamic enhanced MRI and 64 slice spiral CT were performed on all patients.To analyze the value of single dynamic contrast-enhanced MRI(control group)and dynamic contrast-enhanced MRI combined with 64 slice spiral CT(observation group),the image characteristics of erosion degree of each scheme were analyzed.Results(1)Detection rate:A total of 60 positive joints were detected in 50 patients,the detection rates were 93.33%and 83.33%in the observation group and control group,respectively.There was a significant difference between the two groups(P<0.05).(2)Accuracy:Taking the laboratory test results as the gold standard,the accuracy of the observation group(95.00%)was higher than the control group(81.67%).There is a significant difference between the two groups(P<0.05).(3)Image feature judgment of erosion degree:The MRI erosion score was(2.14±0.42),and the CT erosion degree score was(0.56±0.11).There was a significant difference between the two groups(P<0.05).Conclusion Dynamic enhanced MRI combined with 64 slice spiral CT is feasible in the diagnosis of rheumatoid arthritis.It can improve the diagnostic accuracy and judge the degree of erosion of patients accurately.It provides references for clinicians to carry out treatment and is worth of learning.
