Experience of Professor LIU Chinian in Treating Stomachache of Damp Heat Type

刘持年教授认为胃痛病因病机较为复杂,对于湿热型胃痛,先生详于诊察病情,临证多应用辛开苦降法、芳香化湿法以条达中焦气机,恢复脾升胃降之生理功能;五脏一体,故运用清肺开郁法从肺论治脾胃并调摄情志,从肝脾脏腑经络关系着手以调和肝脾疏达气机;脾胃久病,邪气羁留,正气虚损,又气郁而致血瘀,虚实夹杂,故多以调气活血通络法调和气血;强调用药顾护胃气,且重视应用甘平质润之品濡养耗伤之胃阴。先生临证遵循“杂合以治,各得其所宜”的原则,倡导扶正祛邪,主张治疗慢性胃痛注意调摄情志、饮食禁忌和药物煎服方法等,剂型宜汤丸散灵活运用,提高患者依从性。

Professor LIU Chinian holds that the disease cause and mechanism of stomachache are complicated.For stomachache of damp heat type,Professor LIU performs diagnosis carefully.In clinic he uses the methods of pungent dispersing and bitter descending,resolving dampness with aromatics to smooth qi movement of middle energizer,and to restore the physiological function of spleen qi ascending and stomach qi descending.Since five zang organs are closely related,spleen and stomach should be treated from lung using clearing lung to relive depression and emotions should be harmonized,and the liver and stomach should be harmonized to smooth qi based on the connection between liver and spleen meridians.Spleen and stomach disease lasting for long time would cause injury of healthy qi,and qi stagnation would cause blood stasis,manifesting as mixed deficiency and excess.Therefore,Professor LIU usually uses the method of regulating qi,activating blood,unblocking collaterals to harmonize qi and blood.He also emphasizes to protect stomach qi during the treatment,usually using the medicinals of sweet and neutral,of moistening textures to nourish the affected stomach yin.In clinic,he follows the principle of“mixed treatment aiming at various targets”.He advocates tonifying healthy qi to remove evil qi.In the treatment of chronic stomachache,he proposes to pay attention to emotional health,dietary contraindications,and decocting methods.Besides,the dosage forms such as decoction,pill and powder should be applied with flexibility,so as to increase the compliance of patients.
