中药苦参汤熏洗对肛周脓肿患者术后创面愈合及疼痛的影响

Effect of Zhongyao Kushen Decoction Fumigation and Washing on Postoperative Wound Healing and Pain in Patients with Perianal Abscess

目的:探讨肛周脓肿患者术后应用中药苦参汤熏洗的效果。方法:纳入2019年6月至2021年12月收治于新余市中医院的90例肛周脓肿患者,采用随机数字表法分为对照组和观察组,各45例。两组均行切开引流术治疗。对照组术后第2天予以高锰酸钾熏洗,观察组术后第2天行中药苦参汤熏洗,两组均连续治疗14 d。对此两组腐肉脱落时间、伤口愈合时间、创面恢复情况、疼痛程度[疼痛数字评价量表(NRS)]和肛门功能(肛门失禁Wexner评分系统)。结果:观察组腐肉脱落时间(4.03±0.97)d、创面愈合时间(20.03±4.67)均低于对照组的[(5.12±1.11)d、(23.78±5.22)d],创面肉芽组织生长评分(0.89±0.21)分、创面分泌物评分(0.84±0.17)分、水肿评分(0.67±0.11)分、NRS评分(0.61±0.13)分、Wexner评分(3.01±0.65)分均低于对照组的(1.27±0.24)分、(1.30±0.20)分、(0.94±0.21)分、(1.12±0.25)分、(4.37±1.01)分,均差异有统计学意义(均P<0.05)。结论:肛周脓肿患者术后应用中药苦参汤熏洗治疗可促进创面恢复,缩短创口愈合时间,减轻疼痛,改善肛门功能,效果显著,是一种较好的治疗方案。

Objective:To explore the effect of zhongyao kushen decoction fumigation and washing for perianal abscess.Methods:A total of 90 patients with perianal abscess admitted to Xinyu Traditional Chinese Medicine Hospital from June 2019 to December 2021 were enrolled and divided into control group and observation groups by random number table method,with 45 cases in each group.Both groups were treated with incision and drainage.The control group was fumigated with potassium permanganate on the 2nd days after surgery,and the observation group was fumigated with zhongyao kushen decoction on the 2nd day after surgery.Both groups were treated for 14 days.The time of decomposity shedding,wound healing,wound recovery,pain degree[NRS]and anal function(Wexner score for anal incontinence)were compared between the two groups.Results:The shedding time and wound healing time of the observation group were(4.03±0.97)d and(20.03±4.67)d,which were lower than those of the control group[(5.12±1.11)d and(23.78±5.22)d].Wound granulation tissue growth score(0.89±0.21)score,wound secretion score(0.84±0.17)score,edema score(0.67±0.11)score,NRS score(0.61±0.13)score,Wexner score(3.01±0.65)score,Lower than the control group[(1.27±0.24)points,(1.30±0.20)points,(0.94±0.21)points,(1.12±0.25)points,(4.37±1.01)points],the difference was statistically significant(P<0.05).Conclusion:The treatment of perianal abscess with zhongyao kushen decoction fumigation and washing after operation can promote wound recovery,shorten wound healing time,relieve pain and improve anal function,which is a good treatment.
