新生儿缺氧缺血性脑病患儿血清BMP-7,GFAP的表达及意义

Expression and significance of serum BMP-7 and GFAP in neonates with hypoxic-ischemic encephalopathy

摘要 目的 检测新生儿缺氧缺血性脑病(HIE)患儿血清中骨形态发生蛋白-7(BMP-7),血清胶质纤维酸性蛋白(GFAP)水平,并对新生儿进行神经行为测定(NBNA)评分,探讨BMP-7,GFAP水平与脑损伤严重程度的关系。方法 选择75例HIE新生儿作为研究对象(HIE组),选取同时间段产科出生的45例正常新生儿作为对照组。采用酶联免疫吸附法(ELISA)检测血清BMP-7,GFAP水平,对新生儿NBNA评分进行评定。HIE组与对照组之间进行对比分析,比较两组血清相关指标,分析HIE新生儿血清BMP-7,GFAP水平与NBNA评分的相关性。结果 HIE组血清BMP-7,GFAP水平高于对照组(P<0.01);HIE组Apgar评分、NBNA评分低于对照组(P<0.01);HIE组新生儿血清BMP-7,GFAP表达水平与NBNA评分呈负相关(r=-0.776,-0.489,P<0.01),即与缺氧缺血性脑损伤的严重程度呈正相关。结论 联合检测血清BMP-7,GFAP水平有利于临床评估HIE患儿脑损伤严重程度,可为临床HIE的早期防治提供帮助。 Objective To detect serum levels of bone morphogenesis protein-7(BMP-7) and glial fibrillary acidic protein(GFAP) in children with neonatal hypoxic-ischemic encephalopathy(HIE),and to performed the neurobehavioral measurement(NBNA) scores on neonates to explore the relationship between BMP-7 and GFAP levels and the severity of brain injury.Methods A total of 75 neonates born with hypoxic-ischemic encephalopathy in the hospital were selected as subjects(HIE group).Another 45 normal newborns during the same period were selected as the control group.Enzyme-linked immunosorbent assay was used to detect the serum BMP-7 and GFAP levels, furthermore, neonatal behavioral neurological assessment(NBNA) score was analyzed.The serum-related indicators of the two groups were compared between the HIE group and the control group, and the correlation between serum BMP-7,GFAP level and NBNA score of HIE neonates was analyzed.Results Serum BMP-7 and GFAP levels in the HIE group were higher than those in the control group(P<0.01).The Apgar score and NBNA score of the HIE group were lower than those of the control group(P<0.01).Serum BMP-7 and GFAP expression levels in the HIE group were inversely correlated with NBNA scores(r=-0.776,-0.489,P<0.01).Namely, they were positively correlated with the severity of hypoxic-ischemic brain injury.Conclusion The combined detection of serum BMP-7 and GFAP levels is conducive to clinical assessment of the severity of brain injury in children with HIE,and can help the early prevention and treatment of clinical HIE.
