自拟醒脑开窍中药复方联合颈椎牵引治疗颈源性眩晕的效果

The Effect of Self-made Chinese Herbal Medicine for Xingnao Kaiqiao(醒脑开窍)Combined with Cervical Traction on Cervical Vertigo

目的探究自拟醒脑开窍中药复方联合颈椎牵引治疗颈源性眩晕的临床疗效。方法将2019年5月—2019年11月大连市金州区中医医院神经内科收治的颈源性眩晕患者100例,按照随机数表法分为观察组和对照组各50例。对照组予以颈椎牵引疗法,20~30 min/次,隔日1次;观察组在对照组基础加用自拟醒脑开窍方,1剂/d,分3次口服。两组均治疗14 d。比较两组治疗前后眩晕、头痛欲裂、视物旋转、恶心呕吐、肢体麻木、耳鸣积分,神经肽Y(NPY),内皮素1(ET-1),降钙素基因相关肽(CGRP),椎基底动脉血管搏动指数(PI),椎基底收缩期最大流速(VS)及临床总有效率。结果治疗后,观察组眩晕、头痛欲裂、视物旋转、恶心呕吐、肢体麻木、耳鸣积分,NPY、ET-1、PI均小于对照组(P<0.05),两组CGRP水平、VS较治疗前均增加,观察组均大于对照组(P<0.05)。治疗后,观察组有效率92.00%,高于对照组的68.00%,差异有统计学意义(P<0.05)。结论自拟醒脑开窍方联合颈椎牵引治疗颈源性眩晕可有效抑制血栓形成、改善血管痉挛、修复受损细胞、扩张脑血管、加快局部血液运行速率,缓解眩晕、恶心呕吐等症状,提高临床有效率。

Objective To explore the clinical efficacy of self-made Xingnao Kaiqiao(醒脑开窍)combined with cervical traction in the treatment of cervical vertigo.Method From May 2019 to November 2019,100 patients with cervical vertigo treated in the Department of Neurology of Jinzhou District traditional Chinese Medicine Hospital of Dalian were randomly divided into observation group and control group.The control group was treated with cervical traction therapy,20~30min/time,once every other day;The observation group was treated with self-made Xingnao Kaiqiao formula on the basis of the control group,1 dose/d,orally for 3 times.Both groups were treated for 14 days.The vertigo,headache,visual rotation,nausea and vomiting,limb numbness,tinnitus score,neuropeptide Y(NPY),endothelin-1(ET-1),calcitonin gene-related peptide(CGRP),vertebrobasilar artery pulsatility index(PI),maximum velocity of vertebrobasilar systole(VS)and total clinical effective rate were compared between the two groups before and after treatment.Results After treatment,vertigo,headache,visual rotation,nausea and vomiting,limb numbness,tinnitus scores,neuropeptide Y,endothelin-1 and vertebrobasilar artery pulsatility index in the observation group were lower than those in the control group(P<0.05).The level of calcitonin gene-related peptide and the maximum velocity of vertebrobasilar systole in the two groups were higher than those before treatment,The observation group was larger than the control group(P<0.05).After treatment,the effective rate of the observation group was 92.00%,which was higher than 68.00%of the control group(P<0.05).Conclusion Self-made Xingnao Kaiqiao combined with modern medicine to treat cervical vertigo can effectively inhibit thrombosis,improve vasospasm,repair damaged cells,expand cerebral blood vessels,speed up local blood circulation,relieve dizziness,nausea and vomiting,and improve symptoms,clinically effective.
