摘要 目的探讨氨磺必利联合奥氮平治疗难治性精神分裂症(SP)患者外周血淋巴细胞多巴胺转运体(DAT)、多巴胺受体D2(DRD2)水平的影响。方法选取2019年12月至2020年12月在本院确诊的76例SP患者,采用简单随机分组将患者分为观察组和对照组,每组各38例。对照组患者采用奥氮平治疗,观察组采用氨磺必利联合奥氮平治疗,均持续治疗8周。患者临床症状变化采用阳性和阴性症状量表(PANSS)评估;采用逆转录-聚合酶链式反应(RT-PCR)检测外周血淋巴细胞DAT、DRD2水平。观察两组患者不良反应发生情况。结果治疗后,观察组一般精神病理、阴性症状、阳性症状评分均低于对照组,外周血淋巴细胞DAT和DRD2 mRNA水平低于对照组。观察组椎体外系反应、血压降低、失眠等不良反应发生率与对照组比较差异无统计学意义(P>0.05)。结论奥氮平联合氨磺必利治疗能降低外周血DAT、DRD2水平,改善SP患者临床症状。 Objective To investigate the effect of amisulpride combined with olanzapine on the levels of dopamine transporter(DAT)and dopamine receptor D2(DRD2)in peripheral blood lymphocytes of patients with refractory schizophrenia(SP).Methods A total of 76 SP patients diagnosed in our hospital from December 2019 to December 2020 were selected.Simple random grouping was used to divide the patients into observation group and control group,with 38 cases in each group.Patients in the control group were treated with olanzapine,and the observation group was treated with amisulpride combined with olanzapine,and the treatment was continued for 8 weeks.The changes in clinical symptoms of patients were assessed by the Positive and Negative Symptom Scale(PANSS);reverse transcription-polymerase chain reaction(RT-PCR)was used to detect the levels of DAT and DRD2 in peripheral blood lymphocytes.Observe the occurrence of adverse reactions in the two groups of patients.Results After treatment,the scores of general psychopathology,negative symptoms,and positive symptoms in the observation group were lower than those in the control group(P<0.05),and the levels of DAT and DRD2 mRNA in peripheral blood lymphocytes were lower than those in the control group(P<0.05).There was no difference in the incidence of adverse reactions such as extravertebral reactions,blood pressure reduction,and insomnia between the observation group and the control group(P>0.05).Conclusion Olanzapine combined with amisulpride treatment can reduce peripheral blood DAT and DRD2 levels and improve the clinical symptoms of SP patients.
