P27、Th17、Treg、Jab1表达在卵巢子宫内膜异位囊肿中的临床意义

Clinical Significance of P27,Th17,Treg and Jab1 Expression in Ovarian Endometriosis Cyst

目的 研究P27、辅助性T细胞(Th17)、调节性T细胞(Treg)、c-Jun激活区结合蛋白1(Jab1)表达在卵巢子宫内膜异位囊肿中的临床意义,目的在于为临床诊治提供数据支持。方法 将2017年1月1日-2019年5月31日某院接受腹腔镜手术切除治疗的卵巢子宫内膜异位囊肿患者103例以及非卵巢子宫内膜囊肿的良性肿瘤患者96例分别作为观察组和对照组,检测并比较所有受试者卵巢组织的P27、Jab1蛋白表达情况以及外周血Th17、Treg水平。分析P27、Jab1蛋白表达情况和临床病理特征的关系,以及Jab1蛋白表达情况和术后复发、妊娠的关系。结果 观察组P27蛋白阳性率及外周血Treg分别为24.27%(25/103)、4.21±1.04%,均低于对照组的45.83%(44/96)、6.19±1.20%,而Jab1蛋白阳性率及外周血Th17分别为40.78%(42/103)、5.67±1.72%,均高于对照组的26.04%(25/96)、2.44±0.89%。临床分期为Ⅲ期~Ⅳ期卵巢子宫内膜异位囊肿患者的P27蛋白阳性率为12.73%(7/55),低于Ⅰ期~Ⅱ期的37.50%(18/48)。临床分期为Ⅲ期~Ⅳ期卵巢子宫内膜异位囊肿患者的Jab1阳性率为57.18%(32/55),高于Ⅰ~Ⅱ期的20.83%(10/48)。Jab1阳性卵巢子宫内膜异位囊肿患者复发率为19.05%(8/42),高于Jab1阴性患者的4.92%(3/61),且妊娠率为57.14%(24/42),低于Jab1阴性患者的77.05%(47/61)。结论 P27、Treg在卵巢子宫内膜异位囊肿中存在明显低表达,而Th17、Jab1呈明显高表达。此外P27、Jab1表达均和患者临床分期密切相关,且Jab1表达与患者术后复发及妊娠有关。

Objectives To investigate the clinical significance of the expression of P27,helper T cells(Th17),regulatory T cells(Treg) and c-Jun activated region binding protein 1(Jab1) in ovarian endometriosis cysts,in order to provide data support for clinical diagnosis and treatment.Methods From January 1st,2017 to May 31st,2019,103 patients with ovarian endometriosis cyst and 96 patients with benign tumor of non-ovarian endometriosis cyst were included in this study,which were recorded as observation group and control group,respectively.The protein expressions of P27 and Jab1 in ovarian tissues and the levels of Th17 and Treg in peripheral blood of all subjects were detected and compared.The relationship between P27 and Jab1 protein expression and clinicopathological characteristics,as well as the relationship between Jab1 protein expression and postoperative recurrence and pregnancy were analyzed.Results The positive rate of P27 protein and Treg in peripheral blood of the observation group were 24.27%(25/103) and 4.21±1.04%,respectively,which were lower than those of the control group 45.83%(44/96) and 6.19±1.20%.The positive rate of Jab1 protein and Th17 in peripheral blood were 40.78%(42/103) and 5.67±1.72%,respectively,which were higher than those in the control group 26.04%(25/96) and 2.44±0.89%.The positive rate of P27 protein was 12.73%(7/55) in patients withⅢ~Ⅳstage ovarian endometriosis cyst,which was lower than 37.50%(18/48) in patients withⅠ~Ⅱstage ovarian endometriosis cyst.The positive rate of Jab1 was 57.18%(32/55) in patients withⅢ~Ⅳstage ovarian endometriosis cyst,which was higher than that of 20.83%(10/48) in patients withⅠ~Ⅱstage ovarian endometriosis cyst.The recurrence rate of AB1-positive patients with ovarian endometriosis cyst was 19.05%(8/42),which was higher than that of AB1-negative patients 4.92%(3/61),and the pregnancy rate was 57.14%(24/42),which was lower than that of AB1-negative patients 77.05%(47/61).Conclusions The expression of P27 and Treg was significantly lower in ovarian endometriosis cysts,while the expression of Th17 and Jab1 was significantly higher.In addition,the expression of P27 and Jab1 was closely related to the clinical stage of patients,and the expression of Jab1 was related to postoperative recurrence and pregnancy of patients.
