持续质量改进对肾内教学查房质量的影响

The Application of Continuous Quality Improvement in the Nephrology Department Clinical Teaching Rounds

目的 探讨持续质量改进对肾内科教学查房质量的影响,为改进肾内科教学查房质量提供理论依据。方法 以某院肾内科2019年1月至2019年12月临床带教师资为研究对象,对师资的教学查房进行持续质量改进(CQI),将临床师资分为CQI前组及CQI后组,使用某院教学查房评分表评估肾内科师资教学查房水平在CQI前后的变化并进行比较。结果 肾内科师资年龄(45.00±8.52岁VS39.78±6.96岁P=0.156)、教龄(13.11±8.65年VS 7.89±7.56年P=0.180)及高级专业技术职称比例(44.4%VS 22.2%P=0.289)与CQI前类似。与CQI前相比,CQI后,肾内科临床师资的教学查房总分(84.33±6.91VS92.56±3.13 P=0.019)、查房方法评分(39.00±4.17 VS 43.78±2.54 P=0.017)、查房指导技能评分(20.33±3.04 VS 23.78±1.30 P=0.011)及师资教学查房优秀率(33.3%VS 100%P=0.004)均显著改善,差异有统计学意义。结论 在肾内科教学查房中的应用持续质量改进可以显著提高临床带教师资的教学查房技巧,改进师资的查房水平,提升教学查房质量。

Objectives To explore the effect of continuous quality improvement in teaching rounds of nephrology department,and provide a theoretical basis for improving the quality of teaching ward rounds of nephrology department.Methods To study the clinical teachers’ capital in the department of nephrology of a hospital from January 2019 to December 2019.Continuous quality improvement of teachers’ teaching rounds(CQI),The clinical teachers were divided into pre CQI group and post CQI group.A teaching round score table was used to evaluate and compare the changes of teaching round level before and after CQI.Results The teacher’s age(45.00±8.52 years VS 39.78±6.96 years P =0.156),teaching age(13.11±8.65 years VS 7.89±7.56 years P =0.180) and senior professional technical title ratio(44.4% VS 22.2% P =0.289) were similar to those before CQI.After CQI,The total score of teaching round(84.33±6.91 VS 92.56±3.13 P =0.019),the method score(39.00±4.17 VS 43.78±2.54 P =0.017),the instruction skill score(20.33±3.04 VS 23.78±1.30 P =0.011) and the excellent rate of teaching round of clinical teachers in nephrology department(33.3% VS 100% P =0.004) were significantly improved,the difference was statistically significant.Conclusions The application of continuous quality improvement in teaching rounds of nephrology department could significantly improve the teaching ward round skills with teachers,improve the level of teacher ward rounds,and improve the quality of teaching ward rounds.
