某院老年住院患者医疗费用影响因素的研究

Study on the Influencing Factors of Medical Expenses of Elderly Inpatients in a Hospital

目的 通过分析老年患者住院治疗费用及其影响因素,为合理优化住院费用结构提供可靠依据。方法 通过数据收集和分析的方法,对某三甲医院2019年1月1日至2019年9月30日及2021年1月1日至2021年9月30日住院病案首页中60岁及以上老年患者住院治疗费用及其影响因素开展了有关调查。利用卡方检验分析2019年1-9月和2021年1-9月老年患者构成比的差异;采用T检验分析住院费用、自付金额、手术费、药费和材料的差异,统计分析均采用双侧检验。结果 老年患者例均住院费用:2021年1-9月33393.91元;较2019年同期增加1610.71元。患者自付例均费用:2021年1-9月12539.88元;较2019年同期减少1182.85元;药品例均费用2021年1-9月4189.01元较2019年同期减少1009.98元,材料例均费用:2021年1-9月17355.03元,较2019年同期增加620.74元;检查费例均费用:2021年1-9月4685.94元,较2019年同期增加716.30元;手术例均费用:2021年1-9月2031.60元,较2019年同期增加753.90元。结论 老年患者的总住院费用2021年1-9月份年较2019年同期略有增加,自付费用、手术费用、药费、检查费、材料费等项目是老年医疗费用的主要影响因素。

Objective To provide reliable basis for optimizing hospitalization cost structure by analyzing hospitalization cost of elderly patients and its influencing factors.Methods By means of data collection and analysis,the inpatient treatment expenses of elderly patients aged 60 and above in the first page of inpatient medical records from January 1,2019 to September 30,2019 and From January 1,2021 to September 30,2021 in a third grade A hospital and its influencing factors were investigated.Chi-square test was used to analyze the difference of composition ratio of elderly patients between January to September 2019 and January to September 2021.T test was used to analyze the differences in hospitalization expenses,out-of-pocket expenses,surgery costs,drug costs and materials,and statistical analysis was conducted by two-sided test.Results The average hospitalization cost of elderly patients was 33393.91 yuan from January to September 2021;Compared with the same period in 2019,it increased by 1610.71 yuan.Average case cost paid by patients:12539.88 yuan from January to September 2021;Decreased by 1182.85 yuan compared with the same period in 2019;The average case cost of drugs is 4189.01 yuan from January to September 2021,a decrease of 1009.98 yuan compared with the same period in 2019,and the average case cost of materials is 17355.03 yuan from January to September 2021,an increase of 620.74 yuan compared with the same period in 2019;Average inspection fee:4685.94yuan from January to September 2021,an increase of 716.30 yuan over the same period in 2019;Average operation cost:2031.60 yuan from January to September 2021,an increase of 753.90 yuan compared with the same period in 2019.Conclusions The total hospitalization expenses of elderly patients increased slightly from January to September in 2021 compared with the same period in 2019,and out-of-pocket expenses,surgery costs,drug costs,material costs and other items were the main influencing factors of elderly medical expenses.
