新生儿血清维生素D水平对呼吸衰竭的影响 被引量:1

摘要 目的探讨新生儿血清维生素D水平对呼吸衰竭的影响。方法选取2018年6月至2020年6月在本院收治且明确诊断为呼吸衰竭的新生儿149例,同期选取本院出生的健康新生儿208例作为健康对照组,收集并整理两组新生儿的临床资料,包括性别、居住地区、父母年龄、出生体质量、胎龄等基本情况;围产期指标和所患新生儿疾病。结果呼吸衰竭组新生儿血清维生素D水平为21.6(14.60,24.00)μg/L,低于健康对照组25.32(24.00,26.37)μg/L,两组差异有统计学意义(P<0.05);多因素Logistic回归模型分析显示,早产、肺部感染是新生儿发生呼吸衰竭的独立危险因素(P<0.05),5分钟Apgar评分、血清维生素D水平是新生儿发生呼吸衰竭的保护因素(P<0.05)。相关性检验分析结果显示,5分钟Apgar评分和新生儿血清维生素D水平呈正相关(P=0.01)。结论新生儿呼吸衰竭发生与多种因素密切相关,血清维生素D缺乏是其中因素之一。 Objective To investigate the effect of serum vitamin D level on respiratory failure in neonates.Methods Totally 149 neonates with respiratory failure treated in our hospital from June 2018 to June 2020 were selected,and 208 healthy neonates born in our hospital during the same period were selected as the healthy control group.The clinical data of the two groups were collected and sorted,including gender,residential area,age of parents,body mass at birth and gestational age,as well as peripartum indexes and neonatal diseases.Results The serum vitamin D level of the neonates in respiratory failure group was 21.6(14.60,24.00)μg/L,lower than that of healthy control group[25.32(24.00,26.37)μg/L],the difference between the two groups being statistically significant(P<0.05).Multivariate logistic regression analysis showed that preterm birth and pulmonary infection were independent risk factors for neonatal respiratory failure(P<0.05),5-minute Apgar score and serum vitamin D level were protective factors for neonatal respiratory failure(P<0.05).Correlation analysis showed that there was a positive correlation between 5-minute Apgar score and neonatal serum vitamin D level(P=0.01).Conclusion The occurrence of respiratory failure in the newborn is closely related to many factors,and the deficiency of serum vitamin D is one of them.
