摘要
目的 分析2型糖尿病患者对血清肿瘤标志物糖类抗原(CA)199、CA724、铁蛋白(SF)、恶性肿瘤生长因子(TSGF)水平影响的相关因素分析。方法 选取2020年1月至2021年5月在该院住院及门诊治疗的2型糖尿病患者同时检测CA199、CA724、SF、TSGF 142例结果为阳性者作为观察组,另选择同期健康体检且同时检测CA199、CA724、SF、TSGF结果为阴性的体检健康者130例为对照组。收集两组患者的基本信息,如性别、年龄、民族、糖尿病史、体质量等。另再收集实验室相关检查指标如糖化血红蛋白(HbA1c)、空腹血糖(FPG)、三酰甘油(TG)、总胆固醇(TC)以及CA199、CA724、SF、TSGF。比较两组间基础指标、实验室指标差异是否有统计学意义,以其他指标作为自变量,以血清肿瘤标志物水平是否异常作为因变量,观察血清肿瘤标志物与其他指标的相关性。结果 观察组与对照组一般资料比较差异无统计学意义(P>0.05)。观察组与对照组间实验室指标HbA1c、FPG、TG、TC差异有统计学意义(P<0.05)。对照组和观察组CA199、SF、TSGF差异有统计学意义(P<0.05),CA724两组比较差异无统计学意义(P>0.05)。根据性别分组,两组HbA1c、FPG、TG、TC以及CA199、SF、TSGF、CA724比较差异均无统计学意义(P>0.05)。根据年龄分组HbA1c、FPG、TG、TC以及CA199、SF、TSGF差异均有统计学意义(P<0.05)。观察组和对照组HbA1c和CA199、SF呈正相关。结论 SF、CA199升高与HbA1c异常密切相关。如果不能有效控制患者血糖情况,不仅直接影响SF、CA199、TSGF水平,还会增加2型糖尿病患者治疗风险,2型糖尿病患者中如果出现SF、CA199、TSGF呈升高趋势,应考虑对患者进行全面检查,定期观察SF、CA199、TSGF结果的变化,可以有效判断肿瘤标志物是否低浓度良性升高,判断2型糖尿病的控制程度以及是否合并肿瘤的发生。
Objective To analyze the correlation factors of serum tumor markers CA199,CA724,ferritin(SF)and malignant tumor growth factor(TSGF)in patients with type 2 diabetes mellitus.Methods One hundred and forty-two inpatients and outpatients with type 2 diabetes who tested positive for CA199,CA724,SF and TSGF at the same time from January 2020 to may 2021 in Jingdong Medical District of PLA General Hospital were selected as the observation group and 130 healthy subjects who tested negative for CA199,CA724,SF and TSGF at the same time as the control group.Basic information,such as gender,age,ethnicity,diabetes history and weight,was collected from the two groups.In addition,relevant laboratory indicators such as glycosylated hemoglobin(HbAlc),fasting blood glucose(FPG),triglyceride(TG),total cholesterol(TC),CA199,CA724,SF and TSGF were collected.The above indicators(basic indicators and laboratory indicators)were compared between the two groups,and the correlation between serum tumor markers and other indicators was observed with other indicators as independent variables and abnormal serum tumor marker levels as dependent variables.Results There was no statistical difference between the observation group and the control group(P>0.05).There were significant differences in HbAlc,FPG,TG and TC between the observation group and the control group(P<0.05).There were significant differences in CA199,SF and TSGF between the control group and the observation group(P<0.05),but no significant differences in CA724 between the two groups(P>0.05).There were no statistically significant differences in HbAlc,FPG,TG,TC,CA199,SF and TSGF between the two groups according to gender(P>0.05).There were statistically significant differences in HbAlc,FPG,TG,TC,CA199,SF and TSGF among age groups(P<0.05).HbAlc was positively correlated with CA199 and SF in the observation group and the control group.Conclusion The elevation of SF and CA199 was closely related to abnormal HbA1c.If the blood glucose level of patients is not effectively controlled,it will not only directly affect the levels of SF,CA199 and TSGF,but also increase the treatment risk of patients with type 2 diabetes.If SF,CA199 and TSGF show an increasing trend in patients with type 2 diabetes,comprehensive examination should be considered.Regular observation of changes in the results of SF,CA199 and TSGF can effectively determine whether low concentration of tumor markers is benign,the control degree of type 2 diabetes and the occurrence of tumors.
