Effect of time interval between two semen analyses on semen parameters

目的 探讨在不育男性患者两次精液常规检查的时间间隔对精液参数的影响。方法 回顾性分析770份精液样本检查结果,按照两次精液分析时间间隔分为5组,两次精液分析时间间隔≤1个月(A组),1个月<两次精液分析时间间隔≤3个月(B组),3个月<两次精液分析时间间隔≤6个月(C组),6个月<两次精液分析时间间隔≤1年(D组),两次精液分析时间间隔>1年(E组)。分析各组精液样本的精液量、pH值、精子浓度、精子总数、前向运动精子百分率和精子总活力。结果 (1)当两次检查时间间隔在1~3个月、3~6个月时,精子浓度和精子总数与0~1个月、6个月至1年、1年以上3组差异具有统计学意义(P<0.05);而两次检查时间间隔在3~6个月时,前向运动精子百分率和精子总活力与另外4组相比差异具有统计学意义(P<0.05);(2)精液检查次数与液化时间和pH值呈负相关(r=-0.094,P<0.05),与精子前向运动精子百分率和精子总活力呈正相关(r=0.128,P<0.05),检查时间间隔与精子总数、精子前向运动精子百分率和精子总活力呈正相关(r=0.120,P<0.05)。结论 对男性不育症患者进行两次以上精液常规检查分析时,两次检查的时间间隔应该在6个月以内,确保临床医师能够准确诊断患者的病情。

Objective To investigate the effect of the interval of two routine semen examinations on semen parameters in infertile men.Methods The results of 770 semen samples were analyzed retrospectively and divided into 5 groups according to the time interval between two semen analyses.The time interval between two semen analyses≤1 month(group A),1 month<the time interval between two semen analyses≤3 months(group B),3 months<the time interval between two semen analyses≤6 months(Group C),6 months<the time interval between two semen analyses≤1 year(Group D),and the time interval between two semen analyses>1 year(Group E).The semen volume,pH value,sperm concentration,total sperm count,forward motile sperm percentage and total sperm motility of semen samples in each group were analyzed.Results(1)when the interval of two examinations was 1-3 months and 3-6 months,the sperm concentration and total sperm count were significantly different from those in 0-1 month,6 months-1 year and more than 1 year groups,the difference was statistically significant(P<0.05).When the interval between the two examinations was 3-6 months,the percentage of forward motile sperm and total sperm motility were also significantly different from those of the other four groups,the difference was statistically significant(P<0.05).(2)the number of semen examination was negatively correlated with liquefaction time and PH value,the difference was statistically significant(P<0.05),and positively correlated with the percentage of sperm forward motility and total sperm motility,the difference was statistically significant(P<0.05).The interval of semen examination was positively correlated with the total number of sperm,the percentage of sperm forward motility and total sperm motility,the difference was statistically significant(P<0.05).Conclusion When there are more than two routine semen examinations in patients with male infertility,the interval between the two examinations should be less than 6 months to ensure that clinicians can accurately diagnose the patient′s condition.
