2型糖尿病应用达格列净联合二甲双胍的临床疗效及对其血清同型半胱氨酸的影响

Clinical efficacy of dapagliflozin combined with metformin in type 2 diabetes and its effect on serum homocysteine

目的 研究应用达格列净联合二甲双胍治疗2型糖尿病的临床疗效及对其血清同型半胱氨酸的影响。方法 随机选取2019年4月~2021年10月期间就诊并被确诊为2型糖尿病的患者80例,随机分成两组,观察组和对照组各40例,所有患者均行相同常规护理及治疗措施,对照组在此基础上采用二甲双胍治疗,观察组行达格列净联合二甲双胍治疗,对比分析两组患者的治疗有效率、治疗前后的血糖水平(糖化血红蛋白、餐后2h血糖、空腹血糖)及血清同型半胱氨酸的变化和低血糖事件发生率。结果 经综合治疗后,观察组治疗有效率(97.5%)明显高于对照组(82.5%),差异具有统计学意义(P <0.05);与治疗前相比,两组患者的血糖水平均有所降低,观察组患者糖化血红蛋白、餐后2 h血糖、空腹血糖水平降低程度显著优于对照组,差异具有统计学意义(P <0.05);治疗后两组患者血清同型半胱氨酸水平均有不同程度降低,对照组血清同型半胱氨酸水平及低血糖事件发生率高于观察组,差异具有统计学意义(P <0.05)。结论 对2型糖尿病患者采用达格列净联合二甲双胍治疗有效提高了治疗效果,显著降低了患者的血糖水平,同时降低了低血糖并发症发生率,并明显降低了患者血清同型半胱氨酸水平,安全有效。

Objective To study the effect of dapagliflozin combined with metformin on type 2 diabetes and its effect on serum homocysteine.Methods Eighty patients randomly admitted to our hospital who were diagnosed with type 2 diabetes from April 2019 to October 2021 were randomly divided into two groups,40 in the observation group and 40 in the control group.All patients received the same routine care and therapeutic measures:The control group was treated with metformin,and the observation group was treated with dapagliflozin combined with metformin.The treatment efficiency and blood glucose levels before and after treatment were compared between the two groups(glycated hemoglobin,postprandial 2 h blood glucose,fasting blood glucose) and changes in serum homocysteine and incidence of hypoglycemic events.Results After comprehensive treatment,the effective rate(97.5%) in the observation group was significantly higher than that in the control group(82.5%),and the difference was statistically significant(P < 0.05).Compared with before treatment,the blood glucose levels of the two groups were decreased,the observation group patients with HbA1c,2 h postprandial blood glucose,fasting blood glucose levels decreased significantly than the control group,the difference was statistically significant(P < 0.05);serum homocysteine levels were different between the two groups after treatment when the degree was decreased,the serum homocysteine level and the incidence of hypoglycemic events were significantly lower in the observation group than in the control group(P < 0.05).Conclusion The treatment of type 2 diabetes patients with dapagliflozin combined with metformin effectively improved the treatment effect,significantly reduced the blood glucose level of patients,reduced the incidence of hypoglycemic complications,and significantly reduced the serum homocysteine level of patients,which was safe and effective.
