一氧化碳中毒患者血浆和肽素、碳氧血红蛋白和乳酸水平之间的相关性分析 被引量:2

摘要 目的 探讨在一氧化碳(CO)中毒血浆和肽素水平是否发生变化,以及碳氧血红蛋白(COHb)、乳酸与和肽素水平之间是否存在关系。方法选取2018年6月~2021年1月接受治疗的80例因一氧化碳中毒而急诊入院患者,观察组患者一氧化碳中毒接受高流量规范氧治疗并采集血液样本,对照组为健康者仅采集了血液样本。在到达第0、6和12小时收集患者的血液样本,检测并比较分析两组和肽素、乳酸和COHb水平。结果一氧化碳中毒患者在0 h的和肽素水平明显高于健康个体(P <0.05);患者0 h的和肽素水平明显高于6 h、12 h(P <0.05);患者0 h的乳酸水平明显高于6 h、12 h(P <0.05),6 h的乳酸水平明显高12 h(P <0.05);患者0 h的COHb水平明显高于6 h、12 h(P <0.05),6 h的COHb水平明显高于12 h(P <0.05);患者0 h的乳酸水平与碳氧血红蛋白水平呈中度正相关(r=0.551,P <0.001);有短暂意识丧失的患者12 h的和肽素水平和6 h的乳酸水平分别明显高于无短暂意识丧失的患者(P <0.05)。结论 和肽素作为一种应激激素,可用于一氧化碳中毒患者的诊断和监测。然而,和肽素水平并不优于COHb和乳酸水平。 Objective To determine whether plasma and peptidin levels change in carbon monoxide(CO)poisoning,and whether there is a relationship between the levels of carboxyhemoglobin(COHb),lactic acid and peptidin.Methods From June 2018 to January 2021,80 patients with carbon monoxide poisoning who received treatment in our hospital were selected.The observation group received high-flow standardized oxygen treatment for carbon monoxide poisoning and blood samples were collected,while the control group only collected blood samples with serum processing protein level.Blood samples were collected from patients at 0,6,and 12 h to detect and compare the levels of peptidin,lactic acid,and COHb between the two groups.Results The levels of peptide and peptide in carbon monoxide poisoning patients at 0 h were significantly higher than those in healthy subjects(P < 0.05).The levels of peptide at 0 h were significantly higher than those at 6 h and 12 h(P < 0.05).The lactate level at 0 h was significantly higher than that at 6 h and 12 h(P < 0.05),and the lactate level at 6 h was significantly higher than that at 12 h(P < 0.05).COHb level at 0 h was significantly higher than that at 6 h and 12 h(P < 0.05),and COHb level at 6 h was significantly higher than that at 12 h(P < 0.05).There was a moderately positive correlation between the level of lactic acid at 0 h and the level of carboxyhemoglobin(r = 0.551,P < 0.001).The levels of peptide and lactate at 12 h and 6 h in patients with transient loss of consciousness were significantly higher than those in patients without transient loss of consciousness(P < 0.05).Conclusion As a stress hormone,hepeptidin can be used in the diagnosis and monitoring of patients with carbon monoxide poisoning.However,the levels of and peptidin were not superior to those of COHb and lactic acid.
