盐酸纳布啡注射液用于胸腔镜下肺叶切除术后患者PCIA的镇痛效果

Analgesic effect of nalbuphine hydrochloride injection for PCIA after thoracoscopic lobectomy

目的探讨盐酸纳布啡注射液用于胸腔镜下肺叶切除术后患者经静脉自控镇痛(PCIA)的镇痛效果。方法选取2019年3月至2020年2月,牡丹江医学院附属红旗医院收治的90例肺叶切除术患者为研究对象,采用随机数字表法分为观察组(n=45)和对照组(n=45)。术后连接静脉自控镇痛泵,对照组给予盐酸羟考酮注射液,观察组在对照组基础上给予盐酸纳布啡注射液。比较两组患者疼痛情况、睡眠质量、炎症因子水平、不良反应发生率。结果观察组患者术后的视觉模拟量表(VAS)评分低于对照组,差异有统计学意义(P<0.05);观察组患者术后的匹兹堡睡眠质量量表(PSQI)评分低于对照组,差异有统计学意义(P<0.05);观察组患者术后的肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平低于对照组,差异有统计学意义(P<0.05);观察组的不良反应总发生率为4.44%,低于对照组的22.22%,差异有统计学意义(P<0.05)。结论盐酸纳布啡注射液用于胸腔镜下肺叶切除术后患者,可有效缓解疼痛感,改善炎症因子表达,降低用药期间的不良反应,提高患者临床治疗效果及睡眠质量。

Objective To investigate the analgesic effect of nalbuphine hydrochloride injection for patientcontrolled intravenous analgesia(PCIA)after thoracoscopic lobectomy.Methods A total of 90 patients with lobectomy admitted to and treated in Hongqi Hospital Affiliated to Mudanjiang Medical University from March 2019 to February 2020 were selected as research subjects,and they were divided into the observation group(n=45)and the control group(n=45)according to the random number table method.After the operation,the PCIA pump was connected.The control group was treated with oxycodone hydrochloride injection,while the observation group was treated with nalbuphine hydrochloride injection on the basis of the control group.The pain,sleep quality,inflammatory factor level and incidence of adverse reactions(ADRs)were compared between the two groups of patients.Results The postoperative visual analogue scale(VAS)score of the observation group was lower than that of the control group,with statistically significant difference(P<0.05).The scores of the Pittsburgh sleep quality scale(PSQI)in patients of the observation group were lower than those in the control group,with statistically significant differences(P<0.05).The levels of tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).The total incidence of ADRs in the observation group was 4.44%,lower than 22.22%in the control group,with statistically significant difference(P<0.05).Conclusion Nalbuphine hydrochloride injection for patients after thoracoscopic lobectomy can effectively relieve pain,improve the expression of inflammatory factors,reduce ADRs during medication,and improve clinical therapeutic efficacy and sleep quality.
