某三甲医院门诊第二类精神药品使用情况分析

Analysis on the use of classⅡdrugs in outpatient department of a third class A hospital

目的 分析本院门诊2019~2021年第二类精神药品处方,了解本院第二类精神药品使用情况,促进该类药物临床合理使用和规范化管理。方法 从医院信息系统(HIS)系统提取本院门诊药房2019~2021年第二类精神药品的用量、销售额、患者年龄和性别,计算用药频度(DDDs)、日均费用(DDDc)、销售金额排序/药品DDDs排序(B/A)值。结果 2019~2021年本院门诊第二类精神药品处方共15206张,12个品种, 3种剂型(片剂9种,胶囊1种,注射剂2种)。2019~2021年第二类精神药品使用量逐年增长,阿普唑仑片、酒石酸唑吡坦片的DDDs排名始终位列前三名。各药物DDDc基本保持稳定,盐酸曲马多片的DDDc值最大。艾司唑仑片、地西泮针的B/A值均为1.0左右;2020年阿普唑仑片的B/A值最大;2021年酒石酸唑吡坦片的B/A值最小。结论 本院2019~2021年第二类精神药品消耗量显著增加。本院门诊第二类精神药品使用基本合理,少数品种如阿普唑仑、劳拉西泮等可能存在药物滥用风险。医生应规范临床用药,药师也应加强第二类精神药品的处方审核,指导患者合理用药,避免滥用,降低用药风险。

Objective To analyze the prescriptions of classⅡpsychotropic drugs in outpatient department of our hospital from 2019 to 2021,and understand the use of classⅡpsychoactive drugs in our hospital,so as to promote the rational clinical use and standardized management of such drugs.Methods From the Hospital Information System(HIS)system,the dosage,sales,patient age and gender of classⅡpsychotropic drugs in the outpatient pharmacy of our hospital from 2019 to 2021 were extracted,and the defined daily doses(DDDs),defined daily dose cost(DDDc),the ranking ratio of sales amount and DDDs(B/A)were calculated.Results In 2019-2021,there were 15206 outpatient prescriptions of classⅡpsychotropic drugs in the outpatient department of our hospital,with 12 varieties and 3 dosage forms(9 types of tablets,1 type of capsules,and 2 types of injections).In 2019-2021,the use of the classⅡpsychotropic drugs increased year by year,and the DDDs ranking of alprazolam tablets and zolpidem tartrate tablets always ranked among the top three.The DDDc of each drug remained basically stable,and the DDDc value of tramadol hydrochloride was the highest.The B/A values of estazolam tablets and diazepam injections were both around 1.0;the B/A value of alprazolam tablets is the highest in 2020;the B/A value of zolpidem tartrate tablets was the lowest in 2021.Conclusion In 2019-2021,the consumption of classⅡpsychotropic drugs in our hospital increased significantly.The use of the second category of psychotropic drugs in the outpatient department of our hospital is basically reasonable,and a few varieties such as alprazolam and lorazepam may have the risk of drug abuse.Doctors should standardize clinical use of drugs,and pharmacists should also strengthen prescription review of classⅡpsychotropic drugs to guide patients to use them rationally,avoid abuse,and reduce the risk of drug use.
