长时程高电压背根神经节射频脉冲治疗带状疱疹神经痛的疗效

目的 观察长时程高电压背根神经节射频脉冲(PRF)治疗带状疱疹神经痛的疗效.方法 100例胸部急性或亚急性期(≤3个月)带状疱疹,分为普通PRF组(A组)和长时程高电压PRF组(C组),每组各50例,比较两组患者治疗前(T_(0))、治疗后1周(T_(1))、1个月(T_(2))、3个月(T_(3))的疼痛(NRS)评分、睡眠干扰评分(SIS);以及3个月(T_(3))时患者对病情变化的总体印象评分(PGIC)和治疗后的不良反应.结果 两组治疗后各时间点NRS、SIS评分均减低,且C组更明显(P<0.05);C组PGIC评分优于A组(P<0.05);两组患者均未发生严重并发症.结论 长时程高电压背根神经节射频脉冲治疗带状疱疹神经痛,能更有效降低疼痛评分,改善睡眠质量,效果优于普通射频且安全有效.

Objective To investigate the efficacy of high-voltage and long-durarion pulsed radiofrequency(PRF)on dorsal root ganglion in the treatment of herpetic neuralgia(HN).Methods 100 patients with thoracic herpes zoster in acute or subacute phase(W3 months)were randomly divided into the normal PRF group(group A)and the high-voltage and long-duration PRF group(group C),with 50 cases in each group.Changes in pain degree and sleep quality were observed before treatment(T_(0))and at different time points after treatnient(the 1st week(T_(1)),the 1st month(T_(2))and 3rd month(T_(3)).The Numerical rating scale(NRS),sleep interference score(SIS)were used for evaluation of treatment efficacy,and the patient global impression of change(PGIC)and adverse reactions were recorded at the 3rd month(T_(3)).Results The NRS and SIS of three groups decreased significantly at different time points after treatment compared with before treatment and more obvious in group C(P<0.05),the PGIC score in group C was better than group A(P<0.05),no serious complications occurred in the two groups.Conclusion High-voltage and long-durarion pulsed radiofrequency can rapidly alleviate herpetic neuralgia,appropriate to extend the exposure time of PRF,can improve the efficacy and reduce the incidence of PHN.
