高频超声及血流参数诊断甲状腺癌颈部淋巴结转移的效果

High-frequency Ultrasound and Blood Flow Parameters in the Diagnosis of Cervical Lymph Node Metastasis in Thyroid Cancer

目的分析甲状腺癌进行超声血流参数和高频超声检查诊断颈部淋巴结转移的准确性。方法文章总共纳入86例观察对象,均为2018年3月—2021年3月在珠海市中西医结合医院治疗的经手术病理学检查确诊的甲状腺癌患者,根据是否发生颈部淋巴结转移进行分组,分为研究组(淋巴结转移患者,n=45)与对照组(无颈部淋巴结转移患者,n=41),两组患者进行超声血流参数+高频超声检查,观察检查结果。结果在阻力指数(resistance index,RI)、收缩期峰值流速/舒张期流速(systolic/diastolic,S/D)以及搏动指数(pulsatility index,PI)上,研究组明显高于对照组,差异有统计学意义(P<0.05)。从准确率、敏感度和特异度方面进行分析,不同检查方法其结果之间存在着较大差异,联合检测的上述结果均要高于单一高频超声检测,在检出率、阴性、阳性预测值方面,同样是联合检测高于单一检测,差异有统计学意义(P<0.05)。结论超声血流参数+高频超声检查用于甲状腺癌颈部淋巴结转移的效果显著,其敏感度与特异度较高,临床可进一步推广运用。

Objective To analyze the accuracy of ultrasound blood flow parameters and high-frequency ultrasound in the diagnosis of cervical lymph node metastasis for thyroid cancer.Methods A total of 86 observation subjects were included in this article,all of whom were treated in Zhuhai Hospital of Integrated Traditional Chinese and Western Medicine from March 2018 to March 2021 and were diagnosed with thyroid cancer by surgical pathology.They were divided into groups according to whether they had cervical lymph node metastasis or not.They were divided into the study group(patients with lymph node metastasis,n=45)and the control group(patients without cervical lymph node metastasis,n=41),the two groups of patients were underwent ultrasound blood flow parameters+high-frequency ultrasound examination to observe the results.Results In terms of resistance index(RI),systolic/diastolic(S/D)and pulsatility index(PI),the study group was significantly higher than the control group,the differences were statistically significant(P<0.05).From the analysis of accuracy,sensitivity and specificity,there were large differences in the results of different inspection methods.The above results of combined detection were higher than those of single high-frequency ultrasound detection.In terms of detection rate,negative and positive predictive value,the combined detection was also higher than the single detection,and the difference was statistically significant(P<0.05).Conclusion Ultrasound blood flow parameters+high frequency ultrasonography is effective for cervical lymph node metastasis of thyroid cancer,and its sensitivity and specificity are high.It can be further promoted in clinical practice.
