观察炔雌醇环丙孕酮片与地屈孕酮对青春期AUB-O的疗效

Efficacy of Ethinylestradiol Cyproterone Tablets and Dydrogesterone on AUB-O in Adolescence

目的观察炔雌醇环丙孕酮片与地屈孕酮对青春期排卵障碍性异常子宫出血(abnormal uterine bleeding-ovulatory dysfunction,AUB-O)的疗效。方法研究对象选取泉州市妇幼保健院2020年6月—2021年6月间收治的青春期AUB-O患者90例,根据患者接受治疗的方法不同,将其分为对照组(45例)和观察组(45例),分别接受地屈孕酮片和炔雌醇环丙孕酮片治疗。就两组患者的止血效果、不良反应、月经量、经期、月经周期、血清性激素水平进行比较,以探讨其临床疗效。结果观察组的控制出血时间、完全止血时间均显著缩短于对照组,差异有统计学意义(P<0.01);观察组的不良反应发生率显著高于对照组,差异有统计学意义(χ^(2)=4.444,P=0.035);治疗后,对照组的月经量、经期、月经周期均显著优于观察组,差异有统计学意义(P<0.01);治疗后,两组患者血清性激素水平均明显下降,差异有统计学意义(P<0.05);但治疗后两组患者的促卵泡成熟激素(follicle-stimulating hormone,FSH)、黄体生成激素(luteinizing hormone,LH)、血清雌二醇(serum estradiol,E2)水平比较差异无统计学意义(P>0.05)。结论炔雌醇环丙孕酮片和地屈孕酮在青春期AUB-O治疗中,均能改善其临床症状,均有调整经期和月经周期的效果,但相较之下,炔雌醇环丙孕酮片控血止血时间更短,更快促进患者血清性激素水平恢复,但炔雌醇环丙孕酮片的不良反应如头痛、恶心较地屈孕酮常见,因此,临床上应根据患者具体情况选择其治疗方式。

Objective To observe the curative effect of ethinylestradiol cyproterone tablets and dydrogesterone on abnormal uterine bleeding-ovulatory dysfunction(AUB-O)caused by adolescence.Methods The research subjects selected 90 adolescent AUB-O patients admitted to Quanzhou Maternity and Child Health Hospital from June 2020 to June 2021.According to different treatment methods,the patients were divided into the control group(45 cases)and the observation group(45 cases),and received dydrogesterone tablets and ethinyl estradiol cyproterone tablets respectively.The hemostatic effect,adverse reactions,menstrual volume,menstrual period,menstrual cycle,and serum sex hormone levels were compared between the two groups to explore their clinical efficacy.Results The control bleeding time and complete hemostasis time in the observation group were significantly shorter than those in the control group,the difference was statistically significant(P<0.01).The incidence of adverse reactions in the observation group was significantly higher than that in the control group,the difference was statistically significant(χ2=4.444,P=0.035).After treatment,the menstrual volume,menstrual period and menstrual cycle of the control group were significantly better than those of the observation group,the difference was statistically significant(P<0.01).After treatment,serum sex hormone levels in both groups were significantly decreased,the difference was statistically significant(P<0.05).However,after treatment,there was no significant difference in the level of follicle-stimulating hormone(FSH),luteinizing hormone(LH),serum estradiol(E2)of the two groups of patients(P>0.05).Conclusion Ethinylestradiol cyproterone tablets and dydrogesterone can improve the clinical symptoms of adolescent AUB-O treatment,and both have the effect of adjusting menstrual period and menstrual cycle.However,in contrast,ethinyl estradiol cyproterone tablets have a shorter time for hemostasis to control hemostasis and promote the recovery of serum sex hormone levels in patients more quickly.However,the side effects of ethinyl estradiol cyproterone tablets such as dizziness and nausea are more common than dydrogesterone.Therefore,the clinical treatment should be selected according to the specific situation of the patient.
