盐酸右美托咪定在机械通气患者撤机拔管中的应用价值研究

Application Value of Dexmedetomidine Hydrochloride in Weaning and Extubation of Patients With Mechanical Ventilation

目的分析盐酸右美托咪定用于机械通气患者撤机拔管的价值。方法本次研究选择了张家界市人民医院2020年8月—2021年8月收治的132例实施机械通气患者开展研究,根据随机分组方式分成两组,即参照组与试验组,每一组都纳入了66例患者,参照组实施咪达唑仑镇静,试验组实施盐酸右美托咪定镇静,对比两组用药效果。结果试验组的Ramsay镇静评分与参照组对比差异无统计学意义(P>0.05);试验组的镇静起效时间、镇静解除时间、停药后撤机时间、拔管时间均比参照组短,差异有统计学意义(P<0.05);试验组的血气分析指标均优于参照组,差异有统计学意义(P<0.05);试验组的苏醒后躁动发生率及不良反应发生率均低于参照组,差异有统计学意义(P<0.05)。结论在机械通气患者中采取盐酸右美托咪定进行镇静,效果更佳,患者能够更快撤机及拔管,苏醒后躁动发生率较低,安全性高,值得推荐。

Objective To analyze the value of dexmedetomidine hydrochloride in extubation of mechanical ventilation patients.Methods A total of 132 patients who underwent mechanical ventilation in Zhangjiajie People’s Hospital from August 2020 to August 2021 were selected for this study.They were divided into two groups according to the random grouping method,namely,the reference group and the experimental group.Sixty-six patients were included in each group.The patients in the reference group were sedated with midazolam,while those in the experimental group were sedated with dexmedetomidine hydrochloride.The medication effects of the two groups were compared.Results There was no significant difference in Ramsay sedation score between the experimental group and the reference group(P>0.05).The sedation onset time,sedation release time,weaning time after drug withdrawal,and extubation time in the experimental group were all shorter than those in the reference group,and the differences were statistically significant(P<0.05).The blood gas analysis indexes of the experimental group were better than those of the reference group,and the difference was statistically significant(P<0.05).The incidence of agitation and adverse reactions after waking up in the experimental group were lower than those in the reference group,and the difference was statistically significant(P<0.05).Conclusion Dexmedetomidine hydrochloride sedation in patients with mechanical ventilation has better effect,patients can withdraw the machine and extubate faster,the incidence of agitation after recovery is low,and the safety is high,which is worthy of recommendation.
