龈沟液MUC-4、HIF-1α、MCP-1与种植体周围炎ISQ值的关系及联合诊断价值

Relationship between MUC-4,HIF-1α,MCP-1 and peri-implantitis ISQ values in gingival sulcus fluid and their diagnostic value

目的 探讨龈沟液黏蛋白-4(MUC-4)、低氧诱导因子-1α(HIF-1α)、单核细胞趋化蛋白-1(MCP-1)与种植体周围炎(PI)种植体稳定系数(ISQ)值的关系及联合诊断价值。方法 选取华北石油管理局总医院口腔科2018年6月至2020年6月收治的104例因牙列缺损行种植修复患者,其中PI患者70例作为观察组,种植体周围组织健康的患者34例作为对照组。对比两组临床资料、ISQ值、龈沟液MUC-4、HIF-1α、MCP-1水平,分析各指标及其联合检测与ISQ值的关系及对PI的诊断价值。结果 观察组龈沟液MUC-4低于对照组,HIF-1α、MCP-1高于对照组,ISQ值低于对照组,差异均有统计学意义(P<0.05);龈沟液MUC-4与ISQ值呈正相关,HIF-1α、MCP-1与ISQ值呈负相关(P<0.05);龈沟液MUC-4、HIF-1α、MCP-1联合诊断PI的AUC为0.900(95%CI:0.826~0.950),敏感度为85.71%,特异度为82.35%,大于各指标单独诊断(P<0.05);以获取的截断值为界分为低水平与高水平,龈沟液MUC-4低水平患者发生PI的危险度是高水平患者的3.044倍;HIF-1α、MCP-1高水平患者发生PI的危险度是分别是低水平患者的2.008倍、2.400倍。结论 龈沟液MUC-4、HIF-1α、MCP-1与ISQ值显著相关,会显著增加PI风险,且联合检测能取得相对可靠的诊断价值。

Objective To investigate the relationship between gingival crevicular fluid mucin-4(MUC-4),hypoxia-inducible factor-1α(HIF-1α),monocyte chemoattractant protein-1(MCP-1)and periimplantitis(PI)implants Relationship of stability coefficient(ISQ)values and joint diagnostic value. Methods 104 patients admitted to the Department of Dentistry of the General Hospital of North China Petroleum Administration for implant restoration due to tooth loss from June 2018 to June 2020 were selected,among which 70 patients with PI were used as the observation group and 34 patients with healthy peri-implant tissues were used as the control group. The clinical data,ISQ values,gingival sulcus fluid MUC-4,HIF-1α,and MCP-1 levels were compared between the two groups,and the relationship between each indicator and ISQvalues and the diagnostic value of PI was analyzed. Results The MUC-4 of gingival crevicular fluid in the observation group was lower than that in the control group,the HIF-1α and MCP-1 in the observation group were higher than those in the control group,and the ISQ value in the observation group was lower than that in the control group,and the differences were statistically significant(P<0.05). Gingival sulcus fluid MUC-4 was positively correlated with ISQ value,and HIF-1α and MCP-1 were negatively correlated with ISQ value(P<0.05). The AUC of gingival crevicular fluid MUC-4,HIF-1α and MCP-1 for the combined diagnosis of PI was 0.900(95%CI:0.826~0.950),the sensitivity was 85.71%,and the specificity was 82.35%,which was greater than that of each indicator alone(P<0.05). According to the cut-off value obtained,it was divided into low level and high level. The risk of PI in patients with low level of MUC-4 in gingival crevicular fluid was3.044 times that of patients with high level. The risk of PI in patients with high levels of HIF-1α,MCP-1 was2.008 times and 2.400 times higher than that in patients with low levels,respectively. Conclusions Gingival crevicular fluid MUC-4,HIF-1α,and MCP-1 were significantly correlated with ISQ value,which would significantly increase the risk of PI,and the combined detection could obtain relatively reliable diagnostic value.
