针刀治疗腰椎间盘突出症的选点规律研究

Research on the Selection Rule of Acupuncture for the Treatment of Lumbar Disc Herniation

目的:分析针刀治疗腰椎间盘突出症的选点规律,为临床治疗提供参考依据。方法:通过检索中国知网、重庆维普、万方数据库收集自2010年1月-2021年6月临床针刀治疗腰椎间盘突出症相关文献,运用SPSS23.0进行聚类分析,分析针刀处方选点的频次、选点之间的关联规则。结果:共纳入处方45条,选点15个,使用总频次为128次,使用频次最高的选点为棘间韧带、关节突关节、横突间韧带和椎间孔内口。聚类分析结果显示存在3个有效聚类群。治疗频次多为7d/次,3~4次为1个疗程,治疗部位多为腰椎局部选点。结论:针刀治疗腰椎间盘突出症选点配伍的使用有一定规律性,可为临床选点提供借鉴,同时也为选点应用规律的探讨和研究提供新的思路与方法。

Objective:To analyze the selection rule of acupuncture for the treatment of lumbar intervertebral disc herniation,and to provide reference for clinical treatment.Methods:By searching CNKI,Chongqing VIP,and Wanfang databases,we collected relevant literature on clinical acupuncture treatment of lumbar disc herniation from January 2010 to June 2021.Cluster analysis was performed using SPSS 23.0 to analyze the selection of acupuncture prescriptions.The frequency of points and the association rules between selected points.Results:A total of 45 prescriptions were included,15 points were selected,and the total frequency of use was 128.The points with the highest frequency of use were interspinous ligament,facet joint,intertransverse ligament and intervertebral foramen.The cluster analysis results showed that there were 3 valid clusters.The frequency of treatment is mostly 7d/time,3 to 4 times as a course of treatment,and the treatment site is mostly selected from the lumbar spine.Conclusion:The use of acupuncture-knife in the treatment of lumbar intervertebral disc herniation has a certain regularity,which can provide reference for clinical site selection,and also provide new ideas and methods for the discussion and research of the application rules of site selection.
