乙型肝炎患者血清HBV DNA水平变化及其与肝细胞损伤的关系

Changes of serum HBV DNA levels in association with liver function in patients withHepatitis B virus

目的分析乙型肝炎患者血清HBV DNA的水平变化及其与肝细胞损伤的关系。方法根据肝功能生化结果将88例乙型肝炎患者分为轻度组(29例)、中度组(29例)和重度组(30例),血清HBVDNA水平采用实时荧光定量PCR测定,血清总胆汁酸、谷丙转氨酶(ALT)等肝功能指标采用日立生化分析仪测定。比较各组血清HBVDNA、总胆汁酸、ALT水平的差异,分析血清HBV DNA水平与肝功能受损程度的关系。结果血清总胆汁酸、ALT水平重度组明显高于中度组,中度组明显高于轻度组,差异均具有统计学意义(P<0.05)。HBV DNA水平重度组明显高于中度组,中度组明显高于轻度组,差异均具有统计学意义(P<0.05)。结论乙型肝炎患者血清HBV DNA水平越高,其肝功能指标总胆汁酸、ALT水平也相应升高,反应出肝功能损害的加剧,证实HBVDNA水平与肝功能指标具有紧密的关系。

Objective To analyze the changes of serum HBV DNA level in patients with hepatitis B and its relationship with hepatocyte injury.Method According to the biochemical results of liver function,88 patients with hepatitis B were divided into mild group(29 cases),moderate group(29 cases)and severe group(30 cases).Liver function indexes such as total bile acid and alanine aminotransferase(ALT)were measured by Hitachi biochemical analyzer.The differences in serum HBV DNA,total bile acid and ALT levels were compared among the groups,and the relationship between serum HBV DNA levels and the degree of liver function damage was analyzed.Results Serum total bile acid and ALT levels were significantly higher in the severe group than in the moderate group,and significantly higher in the moder-ate group than in the mild group,with statistical significance(P<0.05).The level of HBV DNA in the severe group was significantly higher than that in the moderate group,and the level in the moderate group was significantly higher than that in the mild group,and the differences were statistically significant(P<0.05).Conclusion The level of serum HBV DNA level in hepatitis B patients was consistent with in-crease in the levels of total bile acid and ALT,which reflect the aggravation of liver function damage and confirm that the level of HBV DNA has a close relationship with liver function indicators.
