急性脑卒中患者认知功能障碍的影响因素分析

Analysis of the factors influencing the Cognitive dysfunction in patients with Acute stroke

目的:研究急性脑卒中患者认知功能障碍发生的影响因素,为制定相应的护理方案提供理论依据。方法:按照标准选取213例符合的研究对象,根据病后3个月门诊复诊,使用MoCA量表评分,将研究对象分为认知功能障碍组和非认知功能障碍组,记录两组患者的基本资料,入院生化检查并分析与认知障碍的发生的关系。结果:单因素分析显示年龄、教育程度、饮酒史、糖尿病史、高血压病史、高脂血症、甘油三酯、同型半胱氨酸、卒中分型差异有统计学意义(P<0.05);多因素Logistic回归分析糖尿病、同型半胱氨酸、大动脉粥样硬化型缺血性卒中为急性脑卒中患者发生认知功能障碍的独立危险因素。结论:加强对可能影响认知功能障碍的急性脑卒中患者的健康教育和指导有可能减少患者发生的机率。

Objective: To study the factors of cognitive dysfunction in acute stroke and provide theoretical basis for corresponding nursing plan. Methods: 213 eligible subjects were selected according to the criteria. Using the MoCA scale score, the subjects were divided into cognitive dysfunction and non-cognitive dysfunction groups, recorded the basic data of the two groups, were admitted for biochemical examination and analyzed the relationship with the occurrence of cognitive impairment. Results: Univariate analysis showed significant differences in age, education, alcohol history, diabetes, hypertension, hyperlipidemia, triglyceride, homocysteine, and stroke subtype(P < 0.05);multivariate Logistic regression analysis of diabetes, homocysteine, and stroke subtype of large artery atherosclerosis as the main risk factors for cognitive impairment in patients with acute stroke. Conclusion: Strengthening health education and guidance for acute stroke patients who may affect cognitive impairment has the potential to reduce the chance of occurrence.
