初发癫痫患儿血清骨代谢指标特点分析 被引量:2

摘要 目的 分析初发癫痫儿童血清骨代谢指标的特点。方法 选取2018年至2020年在西北妇女儿童医院神经内科诊断为初发癫痫患儿65例为癫痫组,同时选取在儿童保健科体检的健康儿童100例为对照组。回顾性分析确诊为初发癫痫患儿的临床资料及实验室骨代谢指标{血清25-(OH)维生素D[25-(OH)D]、钙(Ca)、磷(P)和碱性磷酸酶(ALP)},同时从性别、季节(春、夏、秋、冬)和年龄因素多维度分析初发癫痫患儿骨代谢指标的特征;并对两组的相关指标进行比较分析。结果 初发癫痫患儿血清25-(OH)D水平夏季最高、冬季最低,初发癫痫患儿总血清25-(OH)D水平较对照组明显降低(t=3.757,P=0.000),除夏季外,春、秋、冬季初发癫痫患儿血清25-(OH)D水平较对照组均明显降低(t=2.931,P=0.024;t=2.332,P=0.026;t=2.161,P=0.048);初发癫痫患儿总血清Ca水平较对照组明显降低(t=6.235,P=0.000),春季和夏季初发癫痫患儿血清Ca水平较对照组均明显降低(t=9.564,P=0.000;t=3.619,P=0.001);癫痫组患儿总血清P和ALP水平与对照组比较差异均无统计学意义(P>0.05)。0~3岁初发癫痫患儿血清25-(OH)D、Ca水平较对照组均明显降低(t=2.590,P=0.012;t=2.543,P=0.014);4~12岁初发癫痫患儿血清25-(OH)D、Ca水平较对照组也均明显降低(t=4.261,P=0.000;t=6.196,P=0.000);0~3岁、4~12岁初发癫痫患儿血清P和ALP水平与对照组比较差异均无统计学意义(P>0.05)。结论 初发癫痫患儿的25-(OH)D和Ca水平较健康儿童均明显减低。因此,应加强对癫痫患儿的维生素D和Ca水平定期检测评估,并在需要时给予补充适量的维生素D和Ca。 Objective To analyze the characteristics of serum bone metabolism markers in children with new-onset epilepsy.Methods A total of 65 children, who were diagnosed with new-onset epilepsy in the department of neurology, Northwest Women′s and Children′s Hospital between 2018 and 2020,were selected as the epilepsy group.Meanwhile, 100 healthy children conducting physical examinations in the department of children′s health were selected as the control group.The clinical data and laboratory bone metabolism markers [25-hydroxy vitamin D(25-(OH)D),calcium(Ca),phosphorus(P) and alkaline phosphatase(ALP)] of children diagnosed with new-onset epilepsy were retrospectively analyzed.The characteristics of serum bone metabolism markers in children with new-onset epilepsy were analyzed from multiple dimensions, including gender, seasons(spring, summer, autumn and winter) and age.Related markers were compared and analyzed between the two groups.Results The serum 25-(OH)D level of children with new-onset epilepsy was the highest in summer and the lowest in winter, and the total serum 25-(OH)D level of children with new-onset epilepsy was significantly lower than that of the control group(t=3.757,P=0.000).Except for summer, the serum levels of 25-(OH)D of children with new-onset epilepsy in spring, autumn and winter were significantly lower than those of the control group(t=2.931,P=0.024;t=2.332,P=0.026;t=2.161,P=0.048).The total serum Ca level of children with new-onset epilepsy was significantly lower than that of the control group(t=6.235,P=0.000).The serum Ca levels of children with new-onset epilepsy in spring and summer were significantly lower than those of the control group(t=9.564,P=0.000;t=3.619,P=0.001).There were no statistically significant differences in P and ALP levels between the epilepsy group and the control group(P>0.05).The serum 25-(OH)D and Ca levels of 0-3 years old children with new-onset epilepsy were significantly lower than those of the control group(t=2.590,P=0.012;t=2.543,P=0.014),and the serum 25-(OH)D and Ca levels of 4-12 years old children with new-onset epilepsy were also significantly lower than those of the control group(t=4.261, P=0.000;t=6.196,P=0.000).There were no statistically significant differences in serum P and ALP levels among 0-3 years old and 4-12 years old children with new-onset epilepsy and the control group(P>0.05).Conclusion The levels of 25-(OH)D and Ca in children with new-onset epilepsy are significantly lower than those of healthy children.Therefore, regular detection and evaluation of vitamin D and Ca levels in children with epilepsy should be strengthened, and appropriate vitamin D and Ca supplements should be given when necessary.
