天南星的临床应用及其用量探究

通过总结经典名方、名老中医及现代医者临床应用天南星的经验,总结出天南星具有以下特点:1)临床汤剂使用剂量为1.55~45 g,常用量为6~15 g,丸散剂0.08~2.14 g。2)根据疾病、证型、症状,选用天南星的最佳用量与配伍,如天南星燥湿化痰,治疗神经系统疾病(多发性硬化症、发作性咬舌、内耳性眩晕)、消化系统疾病(脂肪肝、顽固性呃逆)、妇科疾病(白带增多)等,天南星用量多在1.55~20 g(汤剂),0.08~2.14 g(丸散);如化痰散结,治疗肿瘤类疾病(脑肿瘤、脑癌、子宫颈癌)、免疫系统疾病(顽固性湿疹)等,汤剂用量多为10~45 g;如化痰行瘀,治疗多囊卵巢综合征,剂量多为10 g。3)根据疾病、证型及症状,配伍相应中药,如天南星燥湿化痰,配伍天麻、苍术、石菖蒲等;如祛风化痰,配伍防风、土鳖虫、威灵仙等;如化痰散结,配伍全蝎、防己、蜈蚣、僵蚕等;如化痰行瘀,配伍桃仁、苍术。

By summarizing the clinical application experience in classic prescriptions and of famous Chinese medicine experts,it is concluded that jackinthepulpit tuber has the following characteristics:1) Its clinical dosage range in decoctions is 1.55-45 g,while its commonly used dosage range is 6-15 g,and in pills or powders 0.08-2.14 g.2) Its best dosage should be selected according to different diseases,syndrome types and symptoms.For example,when jackinthepulpit tuber is used to eliminate dampness and phlegm in the treatment of nervous system diseases (multiple sclerosis,paroxysmal tongue biting,inner ear vertigo),digestive system diseases (fatty liver,intractable hiccup),gynecological diseases (increased leucorrhea),etc.,its dosage range is mostly 1.55-20 g (in decoctions),or 0.08-2.14 g (in pills or powders).For dissipating phlegm and resolving masses to treat tumor diseases (cerebroma,cervical cancer),immune system diseases (refractory eczema),etc.,its dosage range in decoctions is mostly 10-45 g.For dissipating phlegm and eliminating blood stasis in the treatment of the polycystic ovary syndrome,its dosage is mostly 10 g.3) According to varying diseases,syndrome types and symptoms,jackinthepulpit tuber is compatible with corresponding Chinese medicines.For instance,for eliminating dampness and phlegm,it can be combined with tall gastrodia tuber,atractylodes rhizome,grassleaf sweetflag rhizome,etc.For dispelling pathogenic wind and eliminating phlegm,it can be combined with divaricate saposhnikovia root,ground beetle,Chinese clematis root,etc.For dissipating phlegm and resolving masses,it can be combined with scorpion,mealy fangji,centipede,stiffsilkworm,etc.For dissipating phlegm and eliminating blood stasis,it can be combined with peach seed,atractylodes rhizome.
