对比不同氮杂螺酮类抗焦虑药物治疗失眠伴焦虑的效果

Effection of Different Azapirones Anxiolytic Drugs in the Treatment of Insomnia Complicating Anxiety

目的观察不同氮杂螺酮类抗焦虑药物治疗失眠伴焦虑的临床疗效。方法回顾性选取宜昌市优抚医院2018年12月-2021年12月收治的114例失眠伴焦虑患者作为研究对象,根据治疗方法分为A组(n=40)、B组(n=38)和C组(n=36),A组患者接受丁螺环酮治疗,B组患者则接受坦度螺酮治疗,C组患者接受常规治疗。比较3组患者治疗前后匹兹堡睡眠质量指数(Pittsburgh Sleep Quality Index,PSQI)、汉密尔顿焦虑量表(Hamilton Anxiety Scale,HAMA)评分、觉醒次数、觉醒时间及患者治疗期间出现的不良反应。结果治疗前,3组患者以及治疗后A组和B组患者PSQI评分、HAMA评分以及觉醒次数和觉醒时间差异不具有统计学意义(P>0.05);治疗后,3组患者PSQI评分、HAMA评分觉醒次数和觉醒时间均显著低于治疗前,且A组和B组患者显著低于C组(P<0.05);B组患者不良反应发生率5.26%(2/38)显著低于A组22.50%(9/40)和B组25.00%(9/36),(P<0.05)。结论对于失眠伴焦虑患者,坦度螺酮和丁螺环酮均能有效改善患者的睡眠和焦虑情况,但坦度螺酮安全性高于丁螺环酮,不良反应发生率更低。

Objective To compare the clinical efficacy of different azapirones anxiolytic drugs in the treatment of insomnia complicating anxiety.Methods One hundred and fourteen patients with insomnia and anxiety admitted to our hospital from December 2018 to December 2021 were retrospectively divided into group A(n=40),group B(n=38)and group C(n=36)according to different treatment methods.Group C received routine treatment,group A was given buspirone,and group B received tandospirone.Thereafter,Pittsburgh Sleep Quality Index(PSQI),Hamilton Anxiety Scale(HAMA)scores,number of awakenings,awakening time,and adverse effects were compared between the three groups.Results PSQI score,HAMA score,number of awakenings,and the awakening time yielded no statistical difference between the three groups before and after treatment(P>0.05).A decrease was observed in PSQI score,HAMA score,number of awakenings,and the awakening time in all three groups after treatment(P<0.05),and the above four parameters in group A and B were significantly lower than those in the group C(P<0.05).The adverse reaction rate was5.26%(2/38)in group B,which was significantly lower than that in the group A(9/36,22.50%)and group B(9/36,25.00%)(P<0.05).Conclusion In patients with insomnia and anxiety,both tandospirone and buspirone are effective in improving sleep and alleviating anxiety,while tandospirone treatment is safer than buspirone treatment and has a lower incidence rate of adverse effects.
