植入式手臂输液港和胸壁输液港在肺癌化疗患者中的应用比较 被引量:1

Comparison of Implantable Arm Infusion Port and Chest Wall Infusion Port in Patients Undergoing Chemotherapy for Lung Cancer

摘要 目的比较植入式手臂输液港和胸壁输液港在肺癌化疗患者中的应用效果。方法选择肺癌化疗的患者60例,采用随机数字表法将其分为手臂港组(植入手臂式静脉输液港)30例和胸壁港组(植入胸壁式静脉输液港)30例,比较2组患者术后并发症(感染、导管堵塞、血栓和渗液)发生率、术后不同时间点(术后4 h、10 h、第2天和1周)穿刺处疼痛视觉模拟评分法(VAS)评分、术后患者癌因性疲乏评分(包括躯体、情感和认知3个维度)及生活质量评分(包括心理功能、物质生活、社会功能和躯体功能4个维度)。结果手臂港组和胸壁港组患者术后并发症发生率分别为6.66%和13.33%,2组比较差异无统计学意义(P>0.05);手臂港组患者术后4 h、10 h、第2天和1周的穿刺处疼痛VAS评分均低于胸壁港组(P<0.05或P<0.001);手臂港组患者癌因性疲乏评分低于胸壁港组(均P<0.001);手臂港组患者生活质量评分高于胸壁港组(均P<0.001)。结论在肺癌化疗患者中,植入式手臂输液港较植入式胸壁输液港应用效果更好,患者术后疼痛程度和癌因性疲乏程度更低,生活质量更高,不增加并发症发生率。 Objective To compare the application of implantable arm infusion port and chest wall infusion port in patients undergoing chemotherapy for lung cancer.Methods Sixty patients receiving chemotherapy for lung cancer were randomly given venous transfusion via either implantable arm port(30 cases)or chest wall port(30 cases).The incidence of postoperative complications(infection,catheter blockage,thrombosis and exudation),scores of visual analogue scale(VAS)at puncture site,scores of postoperative cancer-related fatigue(body,emotion and cognition),and scores of quality of life(psychological function,material life,social function and physical function)were compared between the two groups.Results There was no significant different in the incidence of complications between the arm port group(6.66%)and the chest wall port group(13.33%)(P>0.05).Compared with the chest wall port group,puncture site VAS scores at 4 hours,10 hour,1 day and 1 week after operation were reduced,cancer-related fatigue scores were decreased,and quality of life scores were increased in the arm port group(P<0.05 or P<0.001).Conclusion Compared chest wall infusion port,implantable arm infusion port reduces postoperative pain and cancer-related fatigue,improves quality of life,and does not increase incidence of complications in patients undergoing chemotherapy for lung cancer.
