摘要
目的 分析外周血白细胞(WBC)、中性粒细胞百分比(NEU%)、血清C反应蛋白(CRP)、血清肝素结合蛋白(HBP)、降钙素原(PCT)等几种常用的炎症标志物在新生儿细菌感染性疾病中的价值。方法 选择2019年1月—2021年1月本院收治且确诊细菌感染的新生儿共91例作为细菌感染组,其中的败血症患者44例作为败血症组,细菌性肺炎患者47例作为细菌性肺炎组。选择本院同期43例非感染性疾病的新生儿作对照组。检测并比较所有研究对象的外周血WBC、NEU%,血清CRP、HBP、PCT水平,同时绘制受试者工作特征曲线(ROC曲线),评价各指标对新生儿细菌感染性疾病的诊断效果。结果 细菌感染组WBC、NEU%、CRP、HBP、PCT均显著高于对照组(P均<0.05);细菌感染组与对照组血小板计数(PLT)相比,无明显差异(P>0.05)。败血症组WBC、CRP均显著高于细菌性肺炎组(P均<0.05),两组NEU%、HBP、PCT、PLT水平无明显差异(P均>0.05)。各指标的ROC曲线显示HBP、PCT对新生儿细菌感染性疾病的诊断效能较好。结论 WBC、NEU%、CRP、HBP、PCT对新生儿细菌感染性疾病的临床诊断具有一定的帮助,其中HBP、PCT诊断效能最好,而WBC和CRP可能在区分新生儿败血症和细菌性肺炎方面具有一定的价值。
Objective To analyze the value of several common inflammatory markers of peripheral blood leukocytes(WBC), neutrophils percentage(Neu%), serum C-reactive protein(CRP), serum heparin binding protein(HBP), procalcitonin(PCT) in neonatal bacterial infectious diseases.Methods Ninety-one neonates diagnosed with bacterial infectious diseases in our hospital during January 2019 and January 2021 were selected as bacterial infection group, including 44 cases of septicemia and 47 cases of bacterial pneumonia. Forty-three neonates with non-infectious diseases were selected as the control group. WBC, NEU% in peripheral blood, CRP, HBP and PCT in serum were detected and compared. ROC curves were drawn to evaluate the diagnostic effect of each index on neonatal bacterial infectious diseases.Results WBC, NEU%, CRP, HBP and PCT in bacterial infection group were significantly higher than those in control group(All P<0.05). There was no significant in platelet count(PLT) between bacterial infection group and control group(P>0.05). WBC and CRP in sepsis group were significantly higher than those in bacterial pneumonia group(P<0.05), while there were no significant differences in NEU%, HBP PCT and PLT between sepsis group and bacterial pneumonia group(P>0.05). ROC curves of all indicators showed that HBP and PCT had better diagnostic efficiency for bacterial infectious diseases in neonates.Conclusions WBC, NEU%, CRP, HBP and PCT are helpful for the clinical diagnosis of neonatal bacterial infectious diseases, among which HBP and PCT have the best diagnostic efficacy, while WBC and CRP may have certain value in distinguishing bacterial pneumonia from sepsis.
