酪酸梭菌肠球菌三联活菌对危重症患者肠道功能的影响

摘要 目的 探讨酪酸梭菌肠球菌三联活菌对危重症患者肠道功能的影响。方法 选择2020年1~12月于浙江省台州医院重症医学科治疗的危重症患者60例为研究对象,随机分为观察组与对照组,每组各30例。所有患者根据具体情况,在入院3 d内逐渐开始肠内营养支持,观察组在此基础上加用酪酸梭菌肠球菌三联活菌治疗。比较两组治疗前后肠道功能障碍与衰竭诊断评分,血清IL-6、CRP、PCT水平及肠道菌群。结果 治疗后,两组肠道功能障碍与衰竭诊断评分较治疗前均显著下降,差异有统计学意义(P<0.05);治疗后观察组肠道功能障碍与衰竭诊断评分显著低于对照组,差异有统计学意义(P<0.05)。治疗后,两组IL-6、CRP、PCT水平均较治疗前显著下降,差异有统计学意义(P<0.05);治疗后观察组IL-6、CRP、PCT水平显著低于对照组,差异有统计学意义(P<0.05)。治疗后,两组肠道菌群粪肠球菌、乳酸杆菌、双歧杆菌水平较治疗前均显著提高,差异有统计学意义(P<0.05);治疗后,观察组肠道菌群粪肠球菌、乳酸杆菌、双歧杆菌水平显著高于对照组,差异有统计学意义(P<0.05)。结论酪酸梭菌肠球菌三联活菌可显著改善危重症肠道功能,改善肠道菌群。 Objective To investigate the effect of clostridium butyricum TO-A on intestinal function in critically ill patients. Methods Sixty critically ill patients treated in our ICU from January to December 2020 were randomly divided into the observation group(n=30) and the control group(n=30). All patients were gradually started on nutritional support within 3 days of admission according to specific circumstances. The observation group was given clostridium butyricum TO-A on this basis. The diagnostic scores of intestinal dysfunction and failure, serum IL-6, CRP, PCT levels, and intestinal bacteria were compared between the two groups before and after treatment. Results After treatment, the diagnostic scores of intestinal dysfunction and failure in the two groups were significantly lower than those before treatment,and the difference had statistical significance(P<0.05). After treatment, the diagnostic scores of intestinal dysfunction and failure in the observation group were significantly lower than those in the control group, and the difference had statistical significance(P<0.05). After treatment, the levels of IL-6, CRP, and PCT in the observation and control groups were significantly decreased compared with those before treatment, and the differences had statistical significance(P<0.05). After treatment,the levels of IL-6,CRP and PCT in the observation group were significantly lower than those in the control group, and the differences had statistical significance(P<0.05).After treatment,the levels of intestinal bacteria Enterococcus faecalis,Lactobacilli,and Bifidobacterium in both groups were significantly increased compared with those before treatment(P<0.05).After treatment, the levels of intestinal bacteria Enterococcus faecalis,Lactobacilli and Bifidobacteria in the observation group were significantly higher than those in the control group, and the differences had statistical significance(P<0.05). Conclusion Clostridium butyricum TO-A can significantly improve intestinal function and intestinal bacteria in critically ill patients.
