乳腺超声分型与血清CEA、CA153、CYFRA21-1水平的相关性分析

摘要 目的探讨乳腺超声分型与血清癌胚抗原(CEA)、糖链抗原153(CA153)、细胞角质蛋白19片段抗原21-1(CYFRA21-1)水平的相关性。方法选取2020年10月至2022年6月在本院进行乳腺超声检查的295例女性为研究对象,其中73例健康者设为对照组,110例良性病变者设为良性病变组,112例乳腺癌患者设为乳腺癌组。采用电化学发光免疫分析法检测血清CEA、CA153、CYFRA21-1水平,采用美国LOGIQ S8彩色多普勒超声诊断仪进行乳腺超声检查。比较三组研究对象的乳腺超声分型、血清CEA、CA153、CYFRA21-1水平,对照组及良性病变组的血清CEA、CA153、CYFRA21-1水平与乳腺超声分型的关系;利用Logistic回归模型分析影响对照组及良性病变组乳腺癌超声分型为致密型、不均型的影响因素。结果乳腺癌组的导管型、混合型占比低于对照组,致密型占比高于对照组(P<0.05);乳腺癌组的混合型、不均型占比低于良性病变组,致密型占比高于良性病变组(P<0.05)。良性病变组、乳腺癌组的血清CEA、CA153、CYFRA21-1水平高于对照组(P<0.05);乳腺癌组的血清CEA、CA153、CYFRA21-1水平高于良性病变组(P<0.05)。混合型、致密型、不均型的血清CEA、CA153、CYFRA21-1水平高于导管型(P<0.05);致密型、不均型的血清CEA、CA153、CYFRA21-1水平高于混合型(P<0.05);不均型的血清CA153、CYFRA21-1水平高于致密型(P<0.05)。单因素Logistic回归分析结果表明,年龄、BMI、哺乳史、CEA、CA153、CYFRA21-1均与对照组及良性病变组乳腺癌超声分型为致密型、不均型有关(P<0.05)。多因素Logistic回归分析结果表明,年龄、BMI、CEA、CA153、CYFRA21-1均是影响对照组及良性病变组乳腺癌超声分型为致密型、不均型的独立危险因素(P<0.05)。结论血清CEA、CA153、CYFRA21-1水平与乳腺超声分型密切相关,是乳腺癌超声分型为致密型、不均型的危险因素,可能能为乳腺癌的早期风险评估提供临床参考依据。 Objective To investigate the correlation between breast ultrasound typing and serum carcinoembryonic antigen(CEA),carbohydrate antigen 153(CA153)and cytokeratin 19 fragment antigen 21-1(CYFRA21-1)levels.Methods A total of 295 women who underwent breast ultrasound examination in our hospital from October 2020 to June 2022 were selected as the research objects.Among them,73 healthy people were set as control group,110 people with benign lesions were set as benign lesion group,and 112 breast cancer patients were set as breast cancer group.The levels of serum CEA,CA153 and CYFRA21-1 were detected by electrochemiluminescence immunoassay,and breast ultrasound was performed by LOGIQ S8 color Doppler ultrasound diagnostic instrument.The relationship between breast ultrasound typing,serum CEA,CA153,CYFRA21-1 levels in the three groups were compared,and relationship between breast ultrasound typing and serum CEA,CA153,CYFRA21-1 levels in the control group and benign lesion group were analyzed;Logistic regression model was used to analyze the influencing factors of dense and uneven breast ultrasound classification in the control group and benign lesion group.Results The proportions of ductal type and mixed type in the breast cancer group were lower than those in the control group,while the proportion of dense type was higher than that in the control group(P<0.05);the proportions of mixed type and uneven type in the breast cancer group were lower than those in the benign lesion group,while the proportion of dense type was higher than that in the benign lesion group(P<0.05).The levels of serum CEA,CA153 and CYFRA21-1 in the benign lesion group and the breast cancer group were higher than those in the control group(P<0.05);the levels of serum CEA,CA153 and CYFRA21-1 in the breast cancer group were higher than those in the benign lesion group(P<0.05).The levels of serum CEA,CA153 and CYFRA21-1 in mixed type,dense type and uneven type were higher than those in ductal type(P<0.05);the levels of serum CEA,CA153 and CYFRA21-1 in dense type and uneven type were higher than those in mixed type(P<0.05);the levels of serum CA153 and CYFRA21-1 in uneven type was higher than that in dense type(P<0.05).The results of univariate Logistic regression analysis showed that age,BMI,breast-feeding history,CEA,CA153 and CYFRA21-1 were all related to the dense and uneven breast ultrasound classification in the control group and the benign lesion group(P<0.05).The results of multivariate Logistic regression analysis showed that age,BMI,CEA,CA153 and CYFRA21-1 were independent risk factors for dense and uneven breast ultrasound classification in the control group and the benign lesion group(P<0.05).Conclusion The levels of serum CEA,CA153 and CYFRA21-1 are closely related to breast ultrasound typing,and are the risk factors for dense and uneven breast ultrasound classification,which may provide clinical reference for early risk assessment of breast cancer.
