PCT、WBC、CRP、ESR联合检测在肺良性疾病中的诊断价值

Diagnostic value of combined detection of PCT,WBC,CRP and ESR in benign lung disease

目的比较降钙素原(PCT)、白细胞计数(WBC)、C反应蛋白(CRP)、红细胞沉降率(ESR)单独及联合检测在肺良性疾病中的诊断价值。方法选取2020年1月至2020年6月于本院呼吸内科收治的350例肺良性疾病患者作为肺良性疾病组,并选取同期110例健康体检者作为健康对照组。比较两组的PCT、WBC、CRP、ESR水平,评估上述4种炎症指标单独及联合检测在肺良性疾病中的诊断价值。结果肺良性疾病组的血清PCT、WBC、CRP、ESR水平均显著高于健康对照组,差异具有统计学意义(P<0.05)。受试者工作特征(ROC)曲线显示,PCT、WBC、CRP、ESR单独检测诊断肺良性病的曲线下面积(AUC)分别为0.527、0.606、0.641、0.574,4种炎症指标联合检测诊断肺良性病的AUC(0.785)高于单独检测;在诊断性试验的检测效能方面,CRP的灵敏度、阴性预测值最高,PCT的特异度较高、阳性预测值最高,联合检测的特异度、准确度最高。结论PCT、WBC、CRP、ESR联合检测在肺良性疾病诊断中较单独检测具有更好的临床应用价值。

Objective To compare the diagnostic values of single and combined detection of procalcitonin(PCT),white blood cell count(WBC),C-reactive protein(CRP)and erythrocyte sedimentation rate(ESR)in benign lung disease.Methods A total of 350 patients with benign lung disease admitted in the respiratory department of our hospital from January 2020 to June 2020 were selected as benign lung disease group,and 110 healthy subjects in the same period were selected as healthy control group.The levels of PCT,WBC,CRP and ESR were compared between the two groups,and the diagnostic values of the above four inflammatory indexes in benign lung disease were evaluated.Results The levels of serum PCT,WBC,CRP and ESR in the benign lung disease group were significantly higher than those in the healthy control group(P<0.05).The receiver operating characteristic(ROC)curve showed that the area under the curve(AUC)of PCT,WBC,CRP and ESR in the diagnosis of benign lung disease were 0.527,0.606,0.641 and 0.574,respectively,and the AUC(0.785)of combined detection of four inflammatory indicators in the diagnosis of benign lung disease was higher than that of single detection;in terms of detection efficiency of diagnostic test,the sensitivity and negative predictive value of CRP were the highest,the specificity of PCT was high and its positive predictive value was the highest,and the specificity and the accuracy of combined detection were the highest.Conclusion The combined detection of PCT,WBC,CRP and ESR has better clinical application value in the diagnosis of benign lung disease than the single detection.
