腹腔镜联合TEM器械高位直肠癌和乙状结肠癌体外根治切除的临床探讨

Clinical Study on Radical Resection of High Rectal Cancer and Sigmoid Colon Cancer with Laparoscopic Combined with Tem Instrument

目的:探讨腹腔镜联合TEM器械在高位直肠癌和乙状结肠癌临床中的应用效果。方法:从2017年1月~2018年1月本院收治的直肠癌和乙状结肠癌患者中选取40例入组研究,所有患者均行体外根治切除术治疗,回顾性分析所有患者临床资料,探讨腹腔镜联合TEM器械在此类疾病临床中的应用价值。结果:所有患者均顺利完成手术,其中2例患者需二次手术治疗,首次手术成功率95.00%;术后所有患者免疫因子TNF-α水平、IL-10水平显著改善,治疗前后对比差异具有统计学意义(P<0.05);同时对所有患者术后并发症情况进行评价,结果显示40例患者中共3例发生术后并发症,并发症发生率7.50%。并于出院后对其展开为期36个月的随访调查,结果显示35例患者3年内未见复发,3例患者3年内复发并迅速恶化,2例患者失访,术后短期存活率92.11%。结论:综合来说,腹腔镜联合TEM器械在直肠癌和乙状结肠癌临床中应用效果确切,安全性和可靠性均可保证,是一种较为理想的手术术式。

Objective:To explore the clinical application of laparoscopy combined with TEM equipment in the clinical application of high rectal cancer and sigmoid colon cancer.Methods:From January 2017 to January 2018 in our hospital,40 patients with rectal cancer and sigmoid colon cancer were selected and enrolled in the study.All patients underwent radical resection in vitro.The clinical data of all patients were retrospectively analyzed and the abdominal cavity was explored.The clinical application value of mirror combined with TEM device in such diseases.Results:All patients completed the operation successfully,2 patients needed second operation,the success rate of the first operation was 95.00%;The levels of immune factors TNF-αand IL-10 in all patients after operation were significantly improved,and the difference between before and after treatment was statistically significant(P<0.05);At the same time,the postoperative complications of all patients were evaluated.The results showed that there were 3 cases of postoperative complications in 40 patients,and the complication rate was 7.50%.The results showed that 35 patients had no recurrence within 3 years,3 patients relapsed within 3 years and deteriorated rapidly,and 2 patients were lost to follow-up.The short-term survival rate was 92.11%.Conclusion:In general,the clinical application of laparoscopy combined with TEM instrument in rectal cancer and sigmoid colon cancer has a definite effect,and the safety and reliability can be guaranteed.It is an ideal surgical procedure.
