加速康复外科护理对宫颈癌患者术后康复影响的Meta分析

Meta-analysis of the effect of accelerated rehabilitation surgical nursing on postoperative rehabilitation of patients with cervical cancer

目的系统评价加速康复外科护理对宫颈癌根治术患者干预的有效性。方法检索Embase、PubMed、Cochrane、中国知网、万方、维普、中国生物医学文献服务系统等数据库,收集自建库至2022年3月有关加速康复外科护理对宫颈癌患者康复效果的随机对照研究(RCT),采用RevMan 5.3软件进行Meta分析。结果共纳入13项RCTs,包含1570例宫颈癌患者。结果显示:与对照组相比,加速康复外科护理能够促进患者术后早期下床[MD=-8.40,95%CI(-11.86,-4.95),P<0.00001]、缩短肛门排气时间[SMD=-2.11,95%CI(-2.75,-1.47),P<0.00001]及排便时间[MD=-1.72,95%CI(-2.33,-1.12),P<0.00001]、缩短患者的住院时间[MD=-2.80,95%CI(-4.15,-1.46),P<0.00001],降低术后并发症发生率[OR=0.31,95%CI(0.23,0.42),P<0.00001]。结论加速康复外科护理应用于宫颈癌根治术围手术期能够促进患者胃肠道功能恢复,减少手术并发症,并缩短住院时间,有利于患者术后康复。

Objective To systematically evaluate the effectiveness of accelerated rehabilitation surgical nursing in the intervention of cervical cancer patients undergoing radical operation.Methods Embase,PubMed,Cochrane,CNKI,Wanfang,VIP,China Biomedical Literature Service system and other databases were searched,and randomized controlled studies(RCT)on the rehabilitation effect of accelerated rehabilitation surgical nursing on cervical cancer patients were collected from the database establishment until March 2022.Meta-analysis was performed with RevMan 5.3 software.Results A total of 13 RCTs were included,including 1570 patients with cervical cancer.The results showed:Compared with the control group,accelerated rehabilitation surgical nursing could promote patients to get out of bed early after surgery[MD=-8.40,95%CI(-11.86,-4.95),P<0.00001,shorten the time of anal exhaust[SMD=-2.11,95%CI(-2.75,-1.47),P<0.00001]and time of defecation[MD=-1.72,95%CI(-2.33,-1.12),P<0.00001],shorten the length of hospital stay[SMD=-2.80,95%CI(-4.15,-1.46),P<0.00001],and decrease the incidence of postoperative complications[OR=0.31,95%CI(0.23,0.42),P<0.00001].Conclusion The application of accelerated rehabilitation surgical nursing in perioperative period of radical cervical cancer can promote the recovery of gastrointestinal function of patients,reduce surgical complications,and shorten the length of hospital stay,which is conducive to postoperative rehabilitation of patients.
