“一全多专”模式在基层县级医院急性脑卒中血管内介入治疗工作中的应用效果

Application of"One Generalist and Multi Specialty"Model in Intravascular Interventional Therapy of Acute Stroke in Grass-roots County-level Hospitals

目的分析“一全多专”模式在基层县级医院急性脑卒中血管内介入治疗工作中的应用效果。方法回顾性分析我院2019年1月至2021年12月采用“一全多专”模式前后共计92例血管内介入治疗颅内大动脉急性闭塞病例,按照是否应用“一全多专”模式分为两组,各46例,分别统计应用前后急性脑卒中血管内介入治疗的各时间节点、术前无创血管影像完成率、术中血压控制达标率、术后颅内出血及脑疝发生率、最终临床获益率等指标,并进行对比。结果应用“一全多专”模式后各时间节点明显缩短,各项指标均有改善,最终临床获益率明显提高。结论“一全多专”模式能够将有限的医疗资源充分整合利用,非常适合运用于基层县级医院开展急性脑卒中血管内介入治疗工作。

Objective To analyze the application effect of"one generalist and multi specialty"model in intravascular interventional therapy of acute stroke in grass-roots county-level hospitals.Methods From January 2019 to December 2021,Wudi County People's Hospital retrospectively analyzed 92 cases of intracranial artery acute occlusion treated by intravascular interventional therapy before and after the"one generalist and multi specialty"model.Divided into two groups according to whether the"one-full-multiple-specialty"mode is applied or not,there were 46 cases in each group.The time nodes of intravascular interventional treatment of acute stroke,the completion rate of preoperative noninvasive vascular imaging,the standard rate of intraoperative blood pressure control,the incidence of postoperative intracranial hemorrhage and cerebral hernia and The final clinical benefit rate before and after the application were compared.Results After the application of"one full and multi specialty"mode,each time node is significantly shortened,various indexes are improved,and the final clinical benefit rate is significantly improved.Conclusion The"one generalist and multi specialty"model can fully integrate and utilize the limited medical resources,and is very suitable for grass-roots county-level hospitals to carry out intravascular interventional therapy for acute stroke.
