足月顺产晚断脐对新生儿胆红素水平的影响分析

Analysis of the Effect of Umbilical Cord Cut off Late in Term Delivery on Neonatal Bilirubin Level

目的评价足月顺产晚断脐对新生儿胆红素水平等方面的影响,为新生儿断脐时间的选择工作提供参考。方法选择我院产科2018年5月至2019年8月足月顺产分娩的新生儿230例。按照断脐时间不同随机分为对照组(采取快速断脐方法)与试验组(采取延迟断脐方法),各115例。对比对照组与试验组新生儿组间血红蛋白、红细胞压积、总胆红素水平以及黄疸情况发生率。结果两组新生儿出生后第1天、第3天HB、HCT水平明显升高,且试验组水平高于对照组,P<0.05;而两组STB水平以及生理性黄疸发生率、病理性黄疸发生率差异无显著性,P>0.05。结论对比快速断脐新生儿,延迟断脐新生儿血液中血红蛋白浓度明显升高,积极预防了新生儿早期贫血风险,但对胆红素水平影响并不明显。

Objective To evaluate the effect of umbilical cord amputation on neonatal bilirubin level,and to provide reference for the selection of umbilical cord amputation time.Methods Two hundred and thirty newborns were selected from our hospital from May 2018 to August 2019.According to the different time of umbilical cutting,they were randomly divided into control group(adopting rapid umbilical cutting method)and experimental group(adopting delayed umbilical cutting method),with 115 cases in each.The hemoglobin,hematocrit,total bilirubin and the incidence of jaundice were compared between the control group and the experimental group.Results The levels of HB and HCT in the two groups of newborns increased significantly on the 1st and 3rd days after birth,and the levels in the experimental group were higher than those in the control group,P<0.05;while the levels of STB,the incidence of physiological jaundice,the incidence of pathological jaundice in the two groups,there was no significant difference in rate,P>0.05.Conclusion Compared with the rapid umbilical cord breaking newborn,the hemoglobin concentration in the blood of the delayed umbilical cord breaking newborn increased significantly,which positively prevented the risk of anemia in the early stage of the newborn.In addition,the effect on bilirubin level was not obvious.
