腹腔镜手术治疗小儿急性化脓性阑尾炎术后不良反应和并发症发生率观察

Observation on the Incidence of Adverse Reactions and Complications After Laparoscopic Operation in Children with Acute Suppurative Appendicitis

目的:研究在小儿急性化脓性阑尾炎中应用腹腔镜手术进行治疗的临床效果。方法:从2019年1月~2019年12月来本院进行治疗的小儿急性化脓性阑尾炎患儿中,选择40例患儿的临床资料作为研究对象。依据随机数字表法分为对照组和实验组,各20例,对照组应用开腹手术进行治疗,实验组实施腹腔镜手术进行治疗。对比两组患儿的手术指标、术前术后应激指标,术前术后炎症指标和术后并发症。结果:实验组患儿围术期的各指标时间值均短于对照组,观察组术中出血量少于对照组,有统计学意义(P<0.05)。术后1d,两组COR、MDA、SOD水平均显著上升,但实验组各项指标数据更低,应激反应更弱,有统计学意义(P<0.05)。术后1d,两组IL-6、IL-8、CRP水平显著提升,有明显炎症反应,但实验组各检测值更低,实验组炎症反应轻于对照组,有统计学意义(P<0.05)。实验组并发症总发生率低于对照组,有统计学意义(P<0.05)。结论:在小儿急性化脓性阑尾炎的治疗中应用腹腔镜手术进行治疗,能有效改善患儿病情,降低术后并发症的发生概率,促进患者术后恢复,增加治疗效果。

Objective:To study the clinical effect of laparoscopic surgery in children with acute suppurative appendicitis.Methods:From January 2019 to December 2019,the clinical data of 40 children with acute suppurative appendicitis were selected as the research object.According to the random number table method,they were divided into control group and experimental group,with 20 cases in each group.The control group was treated with laparotomy,and the experimental group was treated with laparoscopy.The surgical indexes,preoperative and postoperative stress indexes,preoperative and postoperative inflammatory indexes and postoperative complications were compared between the two groups.Results:The perioperative time of each index in the experimental group was shorter than that in the control group,and the amount of intraoperative bleeding in the observation group was less than that in the control group(P<0.05).One day after operation,the levels of cor,MDA and SOD in the two groups increased significantly,but the data of various indexes in the experimental group were lower and the stress response was weaker,which was statistically significant(P<0.05).One day after operation,the levels of IL-6,IL-8 and CRP in the two groups were significantly increased,with obvious inflammatory reaction,but the detection values of the experimental group were lower,and the inflammatory reaction in the experimental group was lighter than that in the control group(P<0.05).The total incidence of complications in the experimental group was significantly lower than that in the control group(P<0.05).Conclusion:The application of laparoscopic surgery in the treatment of acute suppurative appendicitis in children can effectively improve the condition of children,reduce the probability of postoperative complications,promote postoperative recovery of patients,and increase the therapeutic effect.
