摘要
目的:针对慢性阻塞性肺气肿伴有呼吸衰竭患者在治疗中应用无创呼吸机,观察其临床效果。方法:纳入慢阻肺伴有呼吸衰竭患者50例,收治时间为2018年9月~2019年9月,分组原则为随机数字表法,各25例。两组均实施治疗研究,方法分别为常规治疗(对照组)与无创呼吸机辅助治疗(观察组)。通过观察pH值、动脉血氧分压(PaO_(2))、有效率、动脉血二氧化碳分压(PaCO_(2))与并发症情况、生活质量,比较两组的治疗效果。结果:观察组有效率92.00%高于对照组的有效率68.00%(P<0.05)。观察组PaO_(2)在治疗后的数值比治疗前高,且高于对照组,PaCO_(2)在治疗后的数值比治疗前低,且低于对照组,P<0.05;但两组患者治疗后的pH值比较,P>0.05。观察组患者并发症发生率(28.00%)低于对照组患者的并发症发生率(4.00%),有显著差异(P<0.05)。对照组患者生活质量评分显著低于观察组患者,组间对比差异明显(P<0.05)。结论:针对慢阻肺伴有呼吸衰竭患者,治疗中应用无创呼吸机效果突出,作用更明显。
Objective:To observe the clinical effect of non-invasive ventilator in the treatment of chronic obstructive pulmonary emphysema with respiratory failure.Methods:Patients with COPD and respiratory failure(n=50,admission period from September 2018 to September 2019)were enrolled,and the grouping principle was random number table method,25 cases for each.Both groups implemented treatment research,the methods were conventional treatment(control group)and non-invasive ventilator assistance(observation group).By observing the pH value,arterial partial pressure of oxygen(PaO_(2)),effective rate,arterial partial pressure of carbon dioxide(PaCO_(2)),complications,and quality of life,the treatment effects of the two groups were compared.Results:The effective rate of the observation group was 92.00%higher than that of the control group(68.00%)(P<0.05).The value of PaO2 after treatment in the observation group was higher than that before treatment and higher than that in the control group,and the value of PaO2 after treatment was lower than that before treatment and lower than that in the control group,P<0.05;However,the comparison of pH value between the two groups after treatment was P>0.05.The complication rate of the observation group(28.00%)was lower than that of the control group(4.00%),and there was a significant difference(P<0.05).The score of quality of life in the control group was significantly lower than that in the observation group(P<0.05).Conclusion:For patients with chronic obstructive pulmonary disease with respiratory failure,the effect of noninvasive ventilator is prominent and more obvious.
