前瞻性护理中快速康复外科理念在耳鼻喉疾病手术患者的应用效果研究

Study on the Application Effect of the Rapid Recovery Surgical Concept in Predictive Nursing in Patients with ENT Surgery

目的探讨前瞻性护理中快速康复外科理念在耳鼻喉疾病手术患者的应用效果。方法选取2020年8月至2021年11月我院收治的耳鼻喉疾病手术患者86例,采用随机数字表法将患者随机分为两组,各43例。其中,对照组实施前瞻性护理,观察组在对照组的基础上进行快速康复外科理念干预。评估患者遵医嘱情况,完全依从与部分依从之和为总依从率。以我院焦虑自评量表(SAS)与抑郁自评量表(SDS)评估。记录患者抗生素使用与住院时间、知识掌握度。采用我院常用生活质量问卷评估,主要是针对患者睡眠质量、生活乐趣、社会功能及躯体症状4个方面评估,每项25分,分数与生活质量成正比。以我院自拟问卷调查,患者依据护理舒适度进行打分,非常满意率与满意率之和为总满意度。记录患者感染发生例数。结果观察组总依从率为40例(93.02%),对照组为32例(74.42%),观察组总依从率显著高于对照组(χ_(2)=5.460,P<0.05)。干预前两组SAS、SDS评分比较无差异意义(P>0.05),干预后两组SAS、SDS评分均下降,且观察组SAS、SDS评分低于对照组(P<0.05)。观察组抗生素使用及住院时间短于对照组,而知识掌握度评分高于对照组(P<0.05),干预前两组生活质量评分比较无差异意义(P>0.05),干预后两组生活质量评分均升高,且观察组各项评分高于对照组(P<0.05)。结论给予耳鼻喉疾病手术患者基于快速康复外科理念的前瞻性护理有效改善患者预后,缩短住院时间,提高生活质量,值得临床大范围推广应用。

Objective To explore the application effect of the concept of fast recovery surgery in prospective nursing in patients with ENT diseases.Methods A total of 86 patients with ENT diseases who were treated in our hospital from August 2020 to November 2021 were selected,and the patients were randomly divided into two groups by random number table method,with 43 cases in each group.Among them,the control group was given prospective nursing care,and the observation group was given rapid rehabilitation surgery concept intervention on the basis of the control group.The patient's compliance with doctor's orders was assessed,and the total compliance rate was the sum of full compliance and partial compliance.The Self-rating Anxiety Scale(SAS)and the Self-rating Depression Scale(SDS)were used in our hospital.The use of antibiotics,hospitalization time,and knowledge of patients were recorded.The quality of life questionnaires commonly used in our hospital were used to evaluate the quality of life,mainly for the four aspects of sleep quality,joy of life,social function and somatic symptoms,each with 25 points,and the score was proportional to the quality of life.According to the questionnaire survey prepared by our hospital,the patients were scored according to the nursing comfort,and the total satisfaction rate was the sum of the very satisfaction rate and the satisfaction rate.The number of cases of infection in patients was recorded.Results The total compliance rate was 40 cases(93.02%)in the observation group and 32 cases(74.42%)in the control group.The total compliance rate in the observation group was significantly higher than that in the control group(χ_(2)=5.460,P<0.05).There was no significant difference in the SAS and SDS scores between the two groups before the intervention(P>0.05).After the intervention,the SAS and SDS scores in the two groups decreased,and the SAS and SDS scores in the observation group were lower than those in the control group(P<0.05).The antibiotic use and hospital stay in the observation group were shorter than those in the control group,but the score of knowledge mastery was higher than that in the control group(P<0.05).There was no significant difference in the quality of life between the two groups before the intervention(P>0.05).The scores were all increased after the intervention,and the scores in the observation group were higher than those in the control group(P<0.05).Conclusion Prospective nursing based on the concept of fast recovery surgery for patients with ENT diseases can effectively improve the prognosis of patients,shorten the length of hospital stay,and improve the quality of life,which is worthy of widespread clinical application.
