静态进展性牵伸联合传统物理治疗技术治疗老年肘关节挛缩的疗效

摘要 目的探讨静态进展性牵伸联合传统物理治疗技术治疗老年肘关节挛缩的疗效。方法选取2017年1月~2020年6月在杭州市第一人民医院城北院区康复医学科住院治疗的因长期卧床引起的肘关节屈曲挛缩畸形患者76例,随机分为试验组与对照组,每组各38例。对照组予以传统的物理治疗技术进行锻炼,试验组在对照组基础上加静态进展性牵伸治疗,两组疗程均为10周。观察两组治疗前后肘关节主动屈伸时疼痛程度、肘关节活动度(ROM)、肘关节功能和日常生活活动能力评分变化,并比较临床疗效。结果治疗10周后,对照组和试验组VAS评分显著下降,AROM、Mayo评分和ADL评分显著上升(均P<0.05或P<0.01),且试验组变化幅度显著(P<0.05);同时试验组的临床总有效率(94.74%)高于对照组(78.95%)(χ^(2)=4.150,P<0.05)。结论静态进展性牵伸联合传统物理治疗技术治疗老年肘关节挛缩的疗效确切,能明显减轻肘关节主动屈伸时疼痛程度,提高关节活动度,改善关节功能,提高日常生活活动能力。 Objective To investigate the curative effect of static progressive stretching combined with traditional physical therapy in treating elbow contracture in the elderly.Methods Seventy-six patients with elbow flexion contracture deformity caused by long-term bed rest who were hospitalized in the Department of Rehabilitation Medicine in Chengbei Hosptial Area of Hangzhou First People′s Hospital from January 2017 to June 2020 were randomly divided into the experimental group and the control group,with 38 cases in each group.The control group was given traditional physical therapy techniques for exercise,and the experimental group was added with static progressive stretching therapy based on the control group.The course of treatment in both groups was ten weeks.The degree of pain in active flexion and extension of the elbow joint,elbow joint range of motion(ROM),elbow function,and activities of daily living(ADL)were observed before and after treatment,and the clinical efficacy was compared between the two groups.Results After 10 weeks of treatment,the VAS score of the control group and the experimental group was significantly decreased,the AROM,Mayo score and ADL score were significantly increased(all P<0.05 or P<0.01),and the change range of the experimental group was significant(P<0.05);meanwhile,the overall clinical response rate of the experimental group(94.74%)was higher than that of the control group(78.95%)(χ^(2)=4.150,P<0.05).Conclusion Static progressive stretching combined with traditional physical therapy is effective in treating elbow contracture in the elderly.It can significantly reduce the degree of pain during active flexion and extension of the elbow joint,improve joint range of motion,improve joint function and improve activities of daily living.
