rhTPO在自体外周血造血干细胞移植中的临床研究

Clinical study of rhTPO in autologous peripheral blood stem cell transplantation

目的观察rhTPO在自体外周血造血干细胞移植中疗效分析及安全性。方法采用开放、随机、对照的研究,分析2018年9月~2020年5月于宁波大学附属人民医院进行自体造血干细胞移植患者,观察组30例,为应用rhTPO进行自体造血干细胞移植患者:对照组42例,为未应用rhTPO的自体造血干细胞移植患者。观察两组的临床治疗反应、安全性、经济效益。结果观察组血小板植入时间为(11.23±3.38)d,对照组为(12.14±1.59)d,差异有统计学意义(P=0.019)。观察组白细胞植入时间为(11.00±1.20)d,对照组为(11.24±0.96)d,差异无统计学意义(P=0.470)。两组白细胞恢复时间比较,差异无统计学意义(P>0.05)。观察组无因不良反应或不耐受而停药病例,应用rhTPO患者平均住院费用为(60107.19±17890.43)元,对照组平均住院费用为(62895.13±11765.42)元,差异无统计学意义(P>0.05)。结论rhTPO应用于自体造血干细胞移植患者可缩短血小板植入时间,+5 d时应用有效、安全。rhTPO应用不影响白细胞植入时间,观察组平均住院费用更低。

Objective To observe the efficacy and safety of rhTPO in autologous peripheral blood stem cell transplantation.Methods The patients who underwent autologous hematopoietic stem cell transplantation from September 2018 to May 2020 of the Afiliated People′s Hospital of Ningbo University were analyzed.The observation group:30 patients treated with rhTPO,and the control group:42 patients without rhTPO.The clinical treatment response,safety and economic benefit of the two groups were observed.Results The platelet implantation time in the observation group was(11.23±3.38)d and that in the control group was(12.14±1.59)d,the difference was statistically significant(P=0.019).The time of leukocyte implantation in the observation group was(11.00±1.20)d and that in the control group was(11.24±0.96)d,the difference was not statistically significant(P=0.470).There was no significant difference in leukocyte recovery time between the two groups(P>0.05).There were no cases of drug withdrawal due to adverse reactions or intolerance in the observation group.The average hospitalization cost of patients with rhTPO in was(60107.19±17890.43)yuan,and that of patients without rhTPO was(62895.13±11765.42)yuan(P>0.05).Conclusion RhTPO can shorten the time of platelet implantation in patients after autologous hematopoietic stem cell transplantation.It is effective and safe at+5 days.The application of rhTPO did not affect the time of leukocyte implantation.Although there was no difference in hospitalization expenses between the two groups,the average hospitalization expenses of the observation group were lower.
