MRI成像参数对阿尔茨海默症的诊断价值

The diagnostic value of MRI imaging parameters for Alzheimer's disease

目的探讨MRI成像参数对阿尔茨海默症的诊断价值。方法选取浙江省绍兴市第七人民医院2019年6月~2021年3月收治的112例确诊为阿尔茨海默症的患者为研究组,另选取同期于浙江省绍兴市第七人民医院进行体检的志愿者70例为对照组。两组均经MRI成像检查,对比两组的MRI成像参数各向异性分数(FA)、平均扩散率(MD)、径向扩散率(RD),分析FA、MD、RD与同型半胱氨酸(Hcy)、C-反应蛋白(CRP)的相关性及对阿尔茨海默症的诊断价值。结果与对照组相比,研究组FA参数降低,MD、RD参数升高(P<0.05)。重度阿尔茨海默症FA参数最低,MD、RD参数、Hcy、CRP水平最高(P<0.05)。FA、MD、RD、Hcy、CRP与研究组不同病情严重程度相关,且FA、MD、RD与Hcy、CRP相关(P<0.05)。与FA、MD、RD单项诊断相比,三项联合对阿尔茨海默症的诊断价值较高(P<0.05)。结论MRI成像对阿尔茨海默症有较高的诊断价值,且FA、MD、RD与Hcy、CRP相关,有助于阿尔兹海默症的早发现,在疾病的防范、治疗中具有重要作用。

Objective To investigate the diagnostic value of MRI imaging parameters for Alzheimer′s disease.Methods A total of 112 patients with confirmed Alzheimer′s disease who were admitted to Shaoxing 7th People′s Hospital in Zhejiang Province from June 2019 to March 2021 were selected as the study group,and 70 volunteers who underwent physical examination in Shaoxing 7th People′s Hospital in Zhejiang Province during the same period were selected as the control group.Both groups were examined by MRI imaging,and the MRI imaging parameters fractional anisotropy(FA),mean diffusion(MD)and radial diffusion(RD)were compared between the two groups to analyze the correlation between FA,MD and RD with homocysteine(Hcy)and C-reactive protein(CRP)and the diagnostic value for Alzheimer′s disease.Results Compared with the control group,FA parameters decreased,while MD and RD parameters increased in the study group(P<0.05).Patients with severe Alzheimer′s disease had the lowest FA parameters and the highest MD and RD parameters,and Hcy and CRP levels(P<0.05).FA,MD,RD,Hcy and CRP correlated with different disease severity in the study group,and FA,MD and RD correlated with Hcy and CRP(P<0.05).Compared with the single diagnoses of FA,MD,or RD,the combination of the three items had higher diagnostic value for Alzheimer′s disease(P<0.05).Conclusion MRI imaging has a high diagnostic value for Alzheimer′s disease,and FA,MD,RD are associated with Hcy,CRP,contribute to the early detection of Alzheimer's disease,and have an important role in the prevention and treatment of the disease.
