加味苦参汤熏洗坐浴治疗肛周湿疹32例疗效观察

Observation on 32 cases of perianal eczema treated by fumigation and hip bath with Jiawei Kushen decoction

目的观察加味苦参汤熏洗坐浴治疗肛周湿疹32例的临床疗效。方法选取2018年12月~2020年12月于台州市中心医院及浙江中医药大学附属第三医院门诊就诊的肛周湿疹患者64例,将其随机分为2组,每组各32例,对照组予丁酸氢化可的松乳膏外用,治疗组采用加味苦参汤(苦参、防风、露蜂房、甘草、白鲜皮、地肤子各15 g)熏洗坐浴。两组均2周为1个疗程。结果总有效率治疗组为93.75%,对照组为71.88%,两组比较,差异有统计学意义(P<0.05)。治疗后两组患者皮肤瘙痒、肛门渗液、肛周皮损形态等症状积分均有明显下降,且治疗组优于对照组(均P<0.05)。两组嗜酸性粒细胞计数水平较治疗前降低,且治疗组嗜酸性粒细胞计数水平低于对照组(P<0.05)。治疗组及对照组均未见明显不良反应。结论加味苦参汤熏洗坐浴对于肛周湿疹患者疗效确切,能明显改善皮肤瘙痒、肛门渗液、肛周皮肤损害形态等临床症状,减轻患者的痛苦,提高生活质量,且安全可靠,具有独特的优势,值得临床推广应用。

Objective To observe the clinical effect of Jiawei Kushen decoction on 32 cases of perianal eczema.Methods A total of 64 patients with perianal eczema from December 2018 to December 2020 were selected,which were randomly divided into two groups,with 32 cases in each group.The control group was treated by butyric acid cortisol cream,and the treatment group was treated by Jiawei Kushen decoction(Kushen 15 g,Fangfeng 15 g,Lufengfang 15 g,Gancao 15 g,Baixianpi 15 g,Difuzi 15 g)fumigated sitz bath.Two groups were treated for 2 weeks for 1 course.Results The total effective rate was 93.75%in the treatment group and 71.88%in the control group(P<0.05).After treatment,the scores of skin pruritus,anal exudate and perianal skin lesions in the 2 groups all decreased significantly,and the treatment group was better than the control group(P<0.05).The level of Eosinophil granulocyte count in 2 groups was lower than that before treatment(P<0.05),and the count in treatment group was lower than that in control group(P<0.05).There was no adverse reactions in two groups.Conclusion The patients with perianal eczema were treated by sitz bath with Jiawei Kushen decoction shows a definite effect.It can significantly improve clinical symptoms such as skin pruritus,anal exudation and perianal skin damage,reduce patients'pain and improve the quality of life.It is safe and reliable.It has unique advantages and is worthy of clinical application.
