国内乳腺癌术后淋巴水肿器械相关功能锻炼研究热点的共词聚类分析

Co-word cluster analysis of domestic hotspots in study of lymphedema instrument related functional exercise after breast cancer surgery

目的分析国内乳腺癌术后淋巴水肿器械相关功能锻炼的研究现状和热点,为今后研究提供参考。方法检索中国期刊全文数据库、维普中文科技期刊数据库、中国生物医学文献数据库、万方学术论文数据库,提取建库至2020年与乳腺癌术后器械类功能锻炼相关的文献,运用文献计量学方法进行统计分析;应用CiteSpace信息可视化软件对高频关键词进行描述分析和图谱描绘。结果共纳入文献59篇,分布在国内38种期刊,合著的文献48篇,合著率为81.36%;获得基金支持的27篇,资助率为45.76%,其中国家级基金4项,省级基金17项;纳入的文献以随机对照试验为主(64.41%),但多为单中心、小样本的随机试验;“抗阻力训练”“综合消肿疗法”“运动疗法或中医技术联合运动疗法”“康复治疗仪”为该领域研究的热点问题。结论器械类相关的功能锻炼对乳腺癌术后上肢淋巴水肿康复治疗有显著的功效。但该领域的研究存在样本量小、机构单一的问题,缺乏出院后延续性护理的研究及专利的成果转化。今后应开展多中心、大样本、多学科联合的研究,推动护理创新专利成果的推广及转化,并构建适合我国国情的乳腺癌术后上肢淋巴水肿器械类相关的最佳功能锻炼方案及效果评判标准。

Objective To analyze the domestic study status and hotspots in study of lymphedema instrument related functional exercise after breast cancer surgery,and to provide reference for future research.Methods The Chinese Journal Full-text Database,VIP Chinese Science and Technology Journal Database,Chinese Biomedical Literature Database and Wanfang Academic Papers Database were retrieved.The literatures related to lymphedema instrument related functional exercise after breast cancer surgery from the database construction to 2020 were extracted and statistically analyzed by bibliometrics.The CiteSpace information visualization software was used to describe and analyze high-frequency keywords.Results A total of 59 articles were included,distributed in 38 domestic journals,and 48 articles were co-authored,with a co-authored rate of 81.36%.A total of 27 papers were supported by funds,with a funding rate of 45.76%,including 4 national funds and 17 provincial funds.The included literatures were mainly randomized controlled trials(64.41%),but most of them were single-center,small-sample randomized trials.“Resistance training”,“comprehensive detumescence therapy”,“exercise therapy or traditional Chinese medicine combined with exercise therapy”and“rehabilitation therapy instrument”were the hotspots in this field.Conclusion Instrument-related functional exercise has a significant effect on the rehabilitation of upper limb lymphedema after breast cancer surgery.However,the research in this field has the problems of small sample size,single institution,lack of research on postdischarge continuous care and patent achievement transformation.In the future,multi-center,large sample and multidisciplinary joint research should be carried out to promote the popularzation and transformation of nursing innovation patent achievements,and construct the best functional exercise program and effect evaluation standard of upper limb lymphedema after breast cancer surgery that is suitable for China's national conditions.
