Maternal rubella antibody levels among 0-7-month-old infants in Qinghai province in 2018

摘要 目的探讨青海省0-7月龄婴儿风疹母传抗体水平变化。方法采用分层随机抽样方法在青海省8个市州选取未接种含风疹成分疫苗(Rubella containing vaccine,RCV)的0-7月龄婴儿采集血清标本,采用酶联免疫吸附试验检测风疹病毒IgG抗体,分析抗体阳性率和几何平均浓度(Geometric mean concentration,GMC)。结果0-7月龄婴儿血清风疹抗体总阳性率为39.18%(257/656),GMC为13.91IU/m L。0、1、2、3、4、5、6、7月龄婴儿风疹IgG抗体阳性率分别为80.56%、63.01%、43.59%、31.17%、20.73%、22.37%、25.30%、13.92%(χ^(2)=146.28,P<0.01),GMC(IU/m L)分别为63.60、32.48、17.56、15.17、8.77、6.43、6.67、4.24(H=158.01,P<0.01)。多因素Logistic回归分析显示,地区和月龄是影响0-7月龄婴儿风疹抗体阳性率的显著因素,风疹抗体阳性率随着月龄增加而下降(OR=0.62,95%CI:0.57-0.68)。结论0-7月龄婴儿风疹抗体水平随着婴儿月龄增长而下降,大部分婴儿在3月龄以后对风疹易感。建议探讨育龄期妇女RCV接种。 ObjectiveTo explore dynamic changes of maternally-derived rubella antibody among 0-7-month-old infants in Qinghai province.MethodsWe used a stratified random sampling method to select 0-7-month-old infants in 8 prefectures of Qinghai. We collected serum samples and tested for IgG antibody against rubella virus using enzyme-linked immunosorbent assay to determine sero-positivity and geometric mean concentrations(GMCs).ResultsThe overall sero-positive rate of rubella antibody among 0-7-month-old infants was 39. 18%(257/656) with a GMC of 13. 91IU/m L. Positivity rates for 0-,1-, 2-, 3-, 4-, 5-, 6-, and 7-month-old infants were 80. 56%, 63. 01%, 43. 59%, 31. 17%,20. 73%, 22. 37%, 25. 30%, and 13. 92%(χ^(2)= 146. 28, P<0. 01), with GMCs being 63. 60, 32. 48,17. 56, 15. 17, 8. 77, 6. 43, 6. 67, and 4. 24 IU/m L(H= 158. 01, P< 0. 01). Multivariate logistic regression analysis showed region and age were significant factors influencing positivity of rubella antibody among 0-7-month-old infants. Sero-positivity decreased with increasing age( OR = 0. 62, 95%CI:0. 57-0. 68).ConclusionsRubella antibody levels among infants aged 0-7 months decreased with increasing age. Most infants were susceptible to rubella after 3 months of age. We recommend exploring RCV vaccination for women of childbearing-age.
