摘要
目的分析天津市流行性腮腺炎(流腮)经济负担及其影响因素。方法通过中国疾病监测信息报告管理系统选择天津市2018-2019年报告流腮病例进行问卷调查,分析流腮经济负担及其影响因素。结果共纳入流腮病例180例,其中实验室确诊、临床诊断病例分别为106例、74例。流腮病例的总经济负担中位数(四分位数间距)为2108(929-3994)元,其中实验室确诊、临床诊断病例分别为2047(1054-3638)元、2194(649-4441)元(Z=0.01,P=0.971);直接、间接经济负担分别为503(295-997)元、1286(427-2634)元(Z=20.77,P<0.001)。多因素Logistic回归分析显示,直接经济负担住院者高于未住院者(OR=4.76,95%CI:3.13-7.14)、随就诊次数增加而上升(OR=2.37,95%CI:1.69-3.34);间接经济负担住院者高于未住院者(OR=1.79,95%CI:1.49-2.13)。结论天津市流腮经济负担特别是间接经济负担较重;需提高儿童2剂次MuCV接种率,以降低流腮疾病和经济负担。
ObjectiveTo evaluate the economic burden of mumps and factors influencing economic burden in Tianjin.MethodsWe selected mumps cases reported from Tianjin to the China Disease Surveillance Information System during 2018-2019 for a questionnaire-based survey and analyses of the economic burden of mumps and its influencing factors.ResultsA total of 180 mumps cases were included, 106laboratory-confirmed and 74 clinical-confirmed. The median(interquartile range) total economic burden for mumps was 2 108(929-3 994) yuan;laboratory-confirmed and clinical-confirmed cases economic burdens were 2 047( 1 054-3 638) and 2 194( 649-4 441) yuan(Z= 0. 01, P= 0. 971),respectively;direct and indirect economic burdens were 503(295-997) and 1 286(427-2 634) yuan(Z= 20. 77, P<0. 001). Multivariate logistic regression showed that the direct economic burden was higher among hospitalized cases than among non-hospitalized cases(OR = 4. 76, 95%CI: 3. 13-7. 14)and increased with increasing number of medical visits(OR= 2. 37, 95%CI: 1. 69-3. 34). The indirect economic burden was higher among hospitalized cases than among non-hospitalized cases( OR = 1. 79,95%CI: 1. 49-2. 13).ConclusionsThe economic burden of mumps was heavy in Tianjin, especially the indirect burden. We should increase two-dose coverage of mumps containing vaccine to reduce mumps incidence and economic burden.
