摘要
目的 分析天津市健康人群A、C、W135和Y群脑膜炎奈瑟菌(Neisseria meningitidis, Nm)抗体水平。方法 采用分层随机抽样方法于2017-2018年和2020年在天津市4个区选择0-40岁健康人群,采集血清标本,应用酶联免疫吸附试验检测A、C、W135和Y群Nm IgG抗体,分析抗体阳性率(浓度≥0.50μg/m L)、保护率(浓度≥2μg/m L)和几何平均浓度(Geometric mean concentration, GMC)。结果 共纳入1 440名调查对象,A、C、W135、Y群Nm抗体阳性率分别为91.74%、83.13%、73.26%、83.26%,保护率分别为72.15%、46.81%、21.74%、54.24%,GMC(μg/m L)分别为3.54、1.85、1.04、2.08。年龄与A、C、W135、Y群Nm抗体阳性率之间的相关系数(r值)分别为0.050、-0.063、0.016、-0.131(P:0.057、0.017、0.542、<0.001),与保护率之间分别为0.016、-0.055、-0.121、-0.134(P:0.547、0.036、<0.001、<0.001)。脑膜炎球菌疫苗免疫剂次与A、C群Nm抗体阳性率之间的r值分别为0.190、0.111(P:均<0.001),与保护率之间分别为0.087、0.201(P:0.006、<0.001)。结论 天津市健康人群A群和Y群Nm抗体水平较高而C群和W135群相对较低,C群和Y群Nm抗体水平随年龄增长而下降而A群和C群随免疫剂次增加而上升;需探讨适龄儿童多价脑膜炎球菌多糖/结合疫苗免疫策略。
ObjectiveTo determine antibody levels of serogroup A, C, W135 and Y Neisseria meningitidis(Nm) in a healthy population of Tianjin.MethodsWe used a stratified random sampling method to select healthy people aged 0-40 years in 4 districts of Tianjin in 2017-2018 and 2020. We collected serum samples to test for serogroup A, C, W135 and Y Nm IgG antibodies using enzyme-linked immunosorbent assay and determined their positivity rates(concentration≥0. 50μg/m L), protection rates(concentration ≥2μg/m L), and geometric mean concentrations( GMCs).ResultsA total of 1 440 subjects were included. Positivity rates of serogroup A, C, W135, and Y Nm antibodies were 91. 74%, 83. 13%,73. 26%, and 83. 26%, respectively, with protection rates being 72. 15%, 46. 81%, 21. 74%, and 54. 24% and with GMCs being 3. 54, 1. 85, 1. 04, and 2. 08 μg/m L. Correlation coefficients(r values)between age and positivity rates of the four serogroup antibodies were 0. 050,-0. 063, 0. 016, and-0. 131(P: 0. 057, 0. 017, 0. 542, and <0. 001), respectively, with 0. 016,-0. 055,-0. 121, and-0. 134(P: 0. 547, 0. 036, <0. 001, and <0. 001) between age and protection rates. The r values between number of received doses of meningococcal vaccine and positivity rates of serogroup A and C antibodies were 0. 190 and 0. 111(both P<0. 001), respectively, with 0. 087 and 0. 201(P: 0. 006 and <0. 001) between number of received doses and protection rates.ConclusionsThe levels of serogroup A and Y Nm antibodies were high but the levels of serogroup C and W135 Nm were low in healthy people of Tianjin. Serogroup C and Y Nm antibodies decreased with increasing age while serogroup A and C antibodies increased with increasing number of vaccine doses. We should explore immunization strategies with multivalent meningococcal polysaccharide/conjugate vaccines for age-eligible children.
