盐摄入量与高血压病中医证型以及靶器官损害相关性研究

Study on correlation between salt intake and TCM syndrome type and target organ damage degree of hypertension

目的 探讨高血压病患者盐摄入量与中医证型以及心、肾靶器官损害程度的相关性。方法 纳入符合诊断的高血压病患者148例,填写问卷调查,采集临床资料,进行中医辨证分型。通过24小时尿钠排泄量估算每日盐摄入量,测定尿微量白蛋白(MAU)水平,同时应用心脏彩超测定左心室重量指数(LVMI),进行相关性分析。结果 ①盐摄入量越多,血压分级越高。高血压3级组患者盐摄入量明显高于高血压1级组(P<0.05)。②根据盐摄入量分组,高盐组与中盐组SBP、LVMI、MAU明显高于低盐组(P<0.05),高盐组SBP、LVMI、MAU明显高于中盐组(P<0.05)。③高血压中医证型中痰湿壅盛组比例较高;高血压病各中医证型患者盐摄入量比较:肝火亢盛证<痰湿壅盛证<阴阳两虚证<阴虚阳亢证(P<0.05)。④高血压病患者盐摄入量与SBP、LVMI和MAU均呈正相关(P<0.01)。结论 ①盐摄入量与高血压病中医证型相关,随盐摄入量增多,病性由实到虚或虚实夹杂转变。②高血压病患者盐摄入量与SBP、LVMI、MAU呈正相关。

Objective To explore the correlation between salt intake and TCM syndrome type and damage degree of heart and kidney target organs in hypertensive patients.Methods 148 cases of hypertension were selected,and Clinical Data and syndrome differentiation of TCM were collected.Daily salt intake was estimated by 24-hour urinary sodium excretion,and urinary microalbumin(MAU)level was determined.Meanwhile,left ventricular mass index(LVMI)and serum creatinine(Scr),correlation analysis was conducted.Results①With the increase of salt intake,the higher the blood pressure grade.The salt intake of grade 3 hypertension group was significantly higher than that of grade 1 hypertension group(P<0.05).②According to the grouping of salt intake,SBP,LVMI and MAU in high-salt group and medium-salt group were significantly higher than those in low-salt group(P<0.05),SBP,LVMI and MAU in high-salt group were significantly higher than those in medium-salt group(P<0.05).③Comparison of salt intake among different TCM syndrome types of hypertension patients:hyperactive liver fire syndrome<excessive accumulation of phlegm and dampness syndrome<Deficiency of Yin and Yang syndrome<hyperactive Yin and Yang syndrome(P<0.05);The proportion of phlegm-dampness syndrome is high.④Salt intake was positively correlated with SBP,LVMI and MAU in hypertensive patients(P<0.01).Conclusion①Salt intake is related to the TCM syndrome type of hypertension.The proportion of phlegm-dampness syndrome is high.②Salt intake was positively correlated with SBP,LVMI and MAU in hypertensive patients.
